Clinical and Epidemiological Profile of Early Gastric Cancer in a Referral Hospital in Teresina, Piauí

Resumo: Introduction: Early gastric cancer is defined as adenocarcinoma confined to the mucosa or submucosa regardless of lymph node involvement. When diagnosed at this stage, the survival rate at 5 years exceeds 90%. Objective: To characterize the clinical and epidemiological profile of patients with early gastric cancer treated in an oncology referral hospital of Teresina in the period from 2004 to 2009. Method: A descriptive, retrospective and quantitative study was conducted by reviewing medical records with confirmed cases of early gastric cancer treated at the hospital from 2004 to 2009. Results: 22 patients were studied, corresponding to 3.8% of all cases of gastric cancer, 13 of which (59%) were male, whose mean age was 59.7 years. The most common symptom was epigastric pain (n=10, 38.5%). The predominant site was the gastric antrum (n=12, 54%) and the most affected layer was the submucosa (n=14, 63.6%). The the most prevalent degree of differentiation was G3 (n=14, 68.2%). The most recorded macroscopic types were IIc and III in 7 (31%) cases each. The treatment was surgery with total gastrectomy as the most used echnique (n=13, 59%). Conclusion: Early gastric cancer is rarely diagnosed in the area, and it is a fact observed in Western studies and its profile is to those described in the literature.

Cuando este cáncer es diagnosticado en esta etapa, la tasa de supervivencia en 5 años supera el 90%. Objetivo: Caracterizar el perfil clínico y epidemiológico de los pacientes con cáncer precoz tratados en el hospital de referencia en oncología de Teresina, en el período 2004 a 2009. Método: Se realizó un estudio descriptivo, retrospectivo y cuantitativo mediante la revisión de expedientes médicos con asos confirmados de cáncer gástrico precoz tratados en el hospital de 2004 a 2009. Resultados: Se estudiaron 22 pacientes que correspondían a 3,8% de los casos de cáncer gástrico, 13 (59%) eran hombres, la franja etaria fue de 59,7 años. El síntoma más común fue el dolor epigástrico (n=10, 38,5%). La localización predominante fue el antro gástrico (n=12, el 54%) y la capa más afectada fue la submucosa (n=14, 63,6%). El G3 fue el grado de diferenciación más prevalente (n=14, 68,2%). Los tipos macroscópicos más registrados fueron el IIc y III con 7 (31%) casos ada uno. El tratamiento llevado a cabo fue el quirúrgico, y la gastrectomía total fue la técnica más utilizada (n=13, el 59%). Conclusión: El cáncer gástrico precoz es poco diagnosticado y una realidad observada en los estudios occidentales y sigue el perfil semejante a los descritos en la literatura.  Introdução: O câncer gástrico precoce é definido como adenocarcinoma restrito à mucosa ou submucosa independente do comprometimento linfonodal. Quando diagnosticado nesse estágio, a taxa de sobrevida em cinco anos supera 90%. Objetivo: Caracterizar o perfil clinicoepidemiológico dos pacientes com câncer gástrico precoce tratados no Hospital de Referência em Oncologia de Teresina no período de 2004 a 2009. Método: Realizou-se um studo descritivo, retrospectivo e quantitativo através de revisão prontuários com os casos confirmados de câncer gástrico precoce atendidos no hospital de 2004 a 2009. Resultados: Foram estudados 22 pacientes correspondendo a 3,8% de todos os casos de câncer gástrico, sendo 13 (59%) do sexo masculino, a média de idade foi 59,7 anos. O sintoma mais comum foi a epigastralgia (n=10; 38,5%). O local predominante foi o antro gástrico (n=12, 54%) e a camada mais acometida foi a submucosa (n=14, 63,6%). O grau de diferenciação mais prevalente foi o G3 (n=14, 68,2%). Os tipos macroscópicos mais registrados foram o II c e o III com 7 (31%) casos cada. O tratamento realizado foi o cirúrgico com a gastrectomia total a técnica mais utilizada (n=13, 59%).
