Interdisciplinarity and Integrality: the Approach of Social Worker and Nurses in INCA

Resumo: Introduction: Based on the philosophy of expanded clinic, social workers and nursing staff of the outpatient care of the Section of Head and Neck Surgery develop joint activities, systematizing information and actions that complement each other, aiming at integral care for patients with cancer of the larynx. Objectives: To demonstrate the importance of designing expanded clinic in the health-disease process, from the experience of the social worker and nursing with these patients; analyze the actions that facilitate users' access to health care, social and labor, and social security rights; and to describe the outcome of interventions. Method: Cross-sectional study of secondary data of patients with laryngeal cancer with descriptive and inferential statistical analysis. Results: Analyzed 153 cases, mostly male, aged between 46 and 65, white, married, primary education, residents outside Rio, smokers, drinkers, advanced staged patients treated with radiotherapy or surgery with radiotherapy, mostly family providers with autonomous pension bond and family income up to two minimum wages. Nursing actions totaled 19,455. Age and irradiation were significant for care. Conclusion: It demonstrated the importance of extended clinic to produce comprehensive care focused on customers, exerting the interdisciplinarity a fundamental role in triggering strategies to deal with the effects caused by the disease.

Objetivos: Demostrar la importancia de la idea de la Clínica Ampliada en el proceso salud-enfermedad, a partir de la experiencia del trabajador social y de enfermería con estos pacientes; analizar las acciones que faciliten el acceso de los usuarios a los servicios de salud y a los derechos sociales, laborales y a la seguridad social; y describir los resultados de las intervenciones realizadas. Método: Estudio transversal de los datos secundarios de pacientes con cáncer de laringe con el análisis estadístico descriptivo e inferencial. Resultados: Se analizaron 153 casos, en su mayoría varones, de edades comprendidas entre 46 y 65 años, raza blanca, casados, con educación primaria, residentes fuera de Río, fumadores, beben alcohol, etapa avanzada, tratados con radioterapia exclusiva o cirugía con radioterapia, la mayoría es cabeza de familia, con bonos de pensiones autónomo y un ingreso familiar hasta de dos sueldos mínimos. Acciones de enfermería totalizaron 19.455. La edad y la irradiación fueron significativas para recibir los cuidados necesarios. Conclusión: El estudio demostró la importancia de la Clínica Ampliada para producir una atención integral dirigida a los pacientes, donde la interdisciplinariedad ejerció un papel clave en el desencadenamiento de las estrategias para hacer frente a los efectos provocados por la enfermedad.  Introdução: Com base na filosofia de Clínica Ampliada, os assistentes sociais e a equipe de Enfermagem do ambulatório da Seção de Cirurgia de Cabeça e Pescoço desenvolvem atividades conjuntas, sistematizando informações e ações que se complementam, visando à integralidade no atendimento aos pacientes com neoplasias de laringe. Objetivos: Demonstrar a importância da concepção de Clínica Ampliada no processo saúde-doença, a partir da experiência do assistente social e da Enfermagem com esses pacientes; analisar as ações que viabilizem acesso dos usuários à assistência de saúde e aos direitos sociais, previdenciários e trabalhistas; e descrever o resultado das intervenções realizadas. Método: Estudo seccional utilizando dados secundários de portadores de neoplasia laríngea, com análise estatística descritiva e inferencial. Resultados: Analisaram-se 153 casos, a maioria masculina, idade entre 46 e 65 anos, brancos, casados, educação fundamental, moradores fora do Rio, tabagistas, etilistas, estágio avançado, tratados com radioterapia exclusiva ou cirurgia com radioterapia, maioria de provedores da família, com vínculo previdenciário autônomo e renda familiar até dois salários mínimos. Ações de Enfermagem totalizaram 19.455. Idade e irradiação foram significativas para os cuidados.
