Elderly Influence of Gender and Type of Treatment on Nutritional Parameters in Oncology

Resumo: Introduction: The evaluation of the factors that influence on nutritional parameters is an important aspect to be considered in individuals with cancer. Objective: To analyze the clinical, sociodemographic and nutritional status of elderly undergoing cancer treatment, with emphasis on differences in the nutritional indicators, related to the type of treatment and gender. Method: We performed a cross-sectional study with 96 elderly undergoing cancer treatment, which included the application of a sociodemographic and health questionnaire, assessment of nutritional status by Patient-Generated Subjective Global Assessment (PG-SGA) and the Body Mass Index (BMI), assessment food consumption and anthropometric measures. Results: The sample had a mean age of 70,6 years and a predominance of advanced stage disease. The PG-SGA identified moderate or severe malnutrition in 43,8%. According to BMI, 29,1% were underweight and 35,4% overweight. Elderly undergoing chemotherapy compared to those in hormone therapy, showed lower values for almost all anthropometric parameters and higher scores on the PG-SGA. Quantitative values of anthropometric measurements were lower in males, regardless of type of treatment. Conclusion: The type of treatment and gender seem to exert influence on anthropometric measurements, suggesting, in the elderly, higher nutritional depletion in chemotherapy, especially among men. Overweight was observed in the same frequency as normal weight and was equally distributed between treatment types. The identification of nutritional problems is anessential part in addressing the elderly cancer patient.

Objetivo: Analizar el estado clínico, sociodemográficos y nutricional de ancianos que se someten a tratamiento contra el cáncer, con énfasis en las diferencias en los indicadores de riesgo nutricional en relación con el tipo de tratamiento y el género. Método: Estudio transversal con 96 ancianos em tratamiento del cáncer, que incluye la aplicación de un cuestionario sociodemográfico y de la salud, análisis del estado nutricional mediante la Evaluación Subjetiva Global Generada por el Paciente (ESG-GP) y el índice de Masa Corporal (IMC), evaluación del consumo de alimentos y de medidas antropométricas. Resultados: La muestra tenía una edad media de 70,6 años, y el predominio de la enfermedad avanzada. La ESG-GP identificó desnutrición moderada o severa en el 43,8% de los evaluados y, de acuerdo con el IMC, el 29,1% presentaban bajo peso y el 35,4% sobrepeso. La quimioterapia en personas de edad avanzada en comparación con los de la terapia hormonal, mostró valores más bajos para casi todos los parámetros antropométricos y mayor puntuación en la ESG-GP. Fueron identificados valores cuantitativos de las mediciones antropométricas más bajos en los hombres, sin importar el tipo de tratamiento. Conclusión: El tipo de tratamiento y el sexo parecen ejercer influencia en las mediciones antropométricas, lo que sugiere, en los ancianos, un mayor agotamiento nutricional en el tratamiento por quimioterapia, especialmente entre los hombres. El exceso de peso se observó en la misma frecuencia que eutróficos y se distribuyen por igual entre los tipos de tratamiento. La identificación de los problemas de nutrición es una parte esencial en el tratamiento del paciente anciano con cáncer.Introdução: A avaliação dos fatores que exercem influência nos parâmetros nutricionais é um importante aspecto a ser considerado em indivíduos com câncer. Objetivo: Analisar o perfil clínico, sociodemográfico e nutricional de idosos em tratamento oncológico, com ênfase nas diferenças nos indicadores nutricionais relacionados ao tipo de tratamento e ao gênero. Método: Estudo transversal com 96 idosos em tratamento oncológico, que incluiu a aplicação de um questionário sociodemográfico e de saúde, avaliação do estado nutricional pela Avaliação Subjetiva Global Produzida pelo Paciente (ASG-PPP) e pelo índice de Massa Corporal (IMC), avaliação do consumo alimentar e aferição de medidas antropométricas. Resultados: A amostra apresentou idade média de 70,6 anos e predomínio da doença em estágio avançado. A ASG-PPP identificou desnutrição moderada ou grave em 43,8% dos avaliados. Segundo o IMC, 29,1% estavam com baixo peso e 35,4% com excesso de peso. Idosos em quimioterapia, quando comparados aos em hormonioterapia, apresentaram valores inferiores para a quase totalidade parâmetros antropométricos e maior pontua..o na ASG-PPP. Foram identificados valores de medidas antropométricas inferiores no gênero masculino, independente do tipo de tratamento. Conclusão: O tipo de tratamento e o gênero parecem exercer influência nas medidas antropométricas, sugerindo-se, em idosos, maior depleção nutricional no tratamento por quimioterapia, especialmente entre os homens. O excesso de peso foi observado na mesma frequência que a eutrofia e esteve igualmente distribuído entre os tipos de tratamento.
