Resumo: | Data from 286,667 Brazilian Health System (SUS) orders to chemotherapy (QT) and radiotherapy (RT) procedures correspondent to 15,196 cases of female breast cancer treated at the State of Rio de Janeiro, from 1999 to 2003, showed there is a mistake of staging this tumor. The concept of not changing the tumoral stage is not being observed, and the change on staging usually follows a procedure change. Unexpectedly, this mistake was more frequent in comprehensive cancer centers (CACON) than in isolated chemotherapy and/or radiotherapy services. Under the classification of malignant tumor rules, the specialized procedure authorizations (APAC) were analysed. Results point to the revision of the educational programs to medical students, cancer surgeons, clinical oncologists and SUS managers. Thus, it will be possible getting on data to evaluate and compare therapeutic results by tumor staging. |

Thus, it will be possible getting on data to evaluate and compare therapeutic results by tumor staging.A partir da análise de 286.667 autorizações para quimioterapia (QT) e radioterapia (RT) de 15.196 casos de câncer da mama feminina tratados, no Estado do Rio de Janeiro, de 1999 a 2003, evidenciou-se um alto grau de inobservância do conceito de imutabilidade do estádio tumoral, sendo que a mudança do estádio, na maioria das vezes, se relaciona com a mudança de procedimento. Surpreendentemente, este fato é mais preponderante nos estabelecimentos de saúde mais completos como são os hospitais classificados como Centro de Alta Complexidade em Oncologia (CACON), em comparação com os serviços isolados, estritamente ambulatoriais, de quimioterapia ou de radioterapia. Com base nos princípios e regras gerais do estadiamento do câncer e nos dados Sistema Único de Saúde (SUS) relativos a Autorização de Procedimentos de Alta Complexidade (APAC) de QT e RT, analisaram se esses dados e buscou-se contribuir para uma melhor aplicabilidade do Sistema TNM. Ao final, aponta-se para a necessidade de se avaliar como o treinamento e a atualização em sistemas de classificação dos tumores malignos vêm se dando, em todo o Brasil, nas instituições de ensino, no setor de controle e avaliação dos órgãos gestores do SUS e nos encontros científicos dos profissionais que praticam as diversas especialidades médicas envolvidas com o tratamento clínico e cirúrgico do câncer.
