Therapeutic and Diagnostic Procedures to be Followed in Linfomas and Leukemia

Resumo: The authors consider the following diseases as lymphomas — Bryll Symmers — (macrofolicular lymphoma), Hodgkin's disease, retículum-cell sarcoma and Iymphosarcoma. Classifications for Iymphoma and leukemia are given. Besides the classical semiotic means for diagnosis other auxiliary examinations are analvysed such as pathological examination, hematological findings and radiological examination. Among these diagnostic means electrophoresis and boné marrow aspiration are described with greater detail because they represent nowdays a subject of great interest. The different bone marrow findings on lymphomas and leukemia are described. The electrophoretic curve is studied in detail. In this work two types of instruments were used: a) Kern LK 30, Tiselius type which gives electrophoresis with very small amount of serum and b) Wieland-Fischer's apparatus, in which the protein mollecules migration is made over filter paper. Electrophoretic values are also given in the different types of lIymphomas and leukemia. The authors also give the following treatment table of their own: I Treatment by irradiation. II Chimiotherapic treatment. III Surgical treatment. IV Complemenctary treatment. Irradiation treatment is studied in regard to the different radio-active isotopes and also the different roentgenterapeutic tecniques employed in these diseases. Chemiotherapeutic treatment is divided in 4 groups: radiomimetic substances, cytotoxic substances, antimetabolic substances and hormones. In this chapter the doses of the most important drugs used to treat such illnesses are given. As a complementary treatment the authors discuss and give their own experience regarding, exosanguineo transfusion, blood transfusion and antibiotics. Prognosis is given in detail. They refer to several cases of chronic granulocitic leukemia, lymphosarcoma reticulum-cell sarcoma and Hodgkin's disease treated by X-Ray and chemiotherapy with a survival of over 5 years. Numerous photographis, X-ray pictures of patients and also microphotografic findings are given in the article.

Apresentam classificações para os linfomas e leucemias. Para o diagnóstico, além dos meios semióticos clássicos, são analisados os exames subsidiários: anatomia patológica, exames hematológicos e exame radiológico. De todos os elementos diagnósticos apresentados, são estudados com mais detalhe, a eletroforese e o mielograma, por constituirem assunto de grande atualidade. Os vários achados de mielogramas para os linfomas e leucemias, são descritos. O traçado eletroforético é estudado com detalhe. Neste trabalho são apresentados dois tipos de aparelhos: a) Kern LK 30, tipo Tiselius que determinado a eletroforese em mínimas quantidades de sôro; b) tipo Wieland Fischer, cuja migração faz-se no papel de filtro. Apresentam ainda resultados eletroforéticos nos diversos tipos de linfomas e leucemias. Os autores apresentam quadro próprio, no qual dividem o tratamento em 4 grupos distintos, que são: — Tratamento pelas irradiações. Il Tratamento quimioterapico. III — Tratamento cirúrgico. IV — Tratamento auxiliar. Ao relatarem com minúcia o tratamento pelas irradiações, além de tecerem considerações sôbre a terapêutica com os isótopos radioativos, analisam as várias técnicas roentgenterápicas, assim como, suas indicações no tratamento destas afecções. O tratamento quimioterápico é dividido em 4 grupos: substâncias radiomiméticas, substâncias citotóxicas, substâncias anti-metabólicas e hormônios. Neste capítulo são analisadas e relatadas as doses das principais drogas usadas no combate a estas enfermidades. Como tratamento auxiliar, os autores, apresentam considerações e emitem suas experiências sôbre: exo sanguíneo transfusão, transfusão e antibióticos. O prognóstico é apresentado com detalhe. Os autores citam casos de leucemia granulocítica crônica, linfosarcoma, retículo-sarcoma e doença de Hodgkin com sobrevida além de 5 anos após tratamento pelos Raios X e pela quimioterapia.
