PROTOCOL PROPOSAL FOR PREVENTION OF PRESSURE INJURY

Resumo: The pressure injury represents an important problem of health, that even so does not threaten directly the life, represents a great problem for the affected individual, a time that causes them innumerable discomforts and influences in the permanence of the patient in the hospital, making it difficult its social conviviality and increasing the general costs with the treatment. Moreover, the constaint exists that the patient can feel when developing a pressure injury. This reality can be observed on health care institutions that often lack a systematic assistance to the patient in relation prevention of pressure injury, it was decided to create a protocol for prevention and monitoring of pressure injury. After the revision of literature elaborated the protocol, having Scale de Braden (SB), as instrument to diagnosis the patient with risk to develop the pressure injury. The choice for the SB as instrument appraiser if gave for the fact of data in literature to point it as an instrument trustworthy and tested in some realities and also for the fact of the SB to be suitable to the Brazilian reality. Another factor that strengthened the option for the SB is that the lines of direction of the Agency and Quality will be Health Care Research (AHCPR), recommends the SB for risk evaluation to develop pressure injury. One expects that in the adoption of the protocol of assistance to patients with UD, the health care institutions they find subsidies for systematization of this assistance, thus making possible the elaboration of the diagnosis, the lapsing of cares, the register of essential data for the treatment and evolution of the assistance, favoring the communication between health staff attending the patient.

Além disso, existe o constrangimento que o paciente pode sentir ao desenvolver uma LPP. Essa realidade poder ser observada nas instituições de saúde que muitas vezes não possuem uma sistematização de assistência ao paciente em relação à prevenção da UD, resolveu-se criar um protocolo de prevenção e monitorização da LPP.Após a revisão da literatura elaborou-se o protocolo, tendo a Escala de Braden (EB), como instrumento para diagnosticar o paciente com risco de desenvolver a UD. A escolha pela EB como instrumento avaliador se deu pelo fato de dados na literatura apontá-la como um instrumento confiável e testado em várias realidades e também pelo fato da EB estar adaptada à realidade brasileira. Outro fator que reforçou a opção pela EB é que as diretrizes da Agency for Health Care Research and Quality (AHCPR), recomendam a EB para avaliação de risco de desenvolver LPP.
