DEPRESSÃO PÓS-PARTO EM PUÉRPERAS PRIMÍPARAS E AS INTERAÇÕES COM A CONJUGALIDADE

Resumo: During pregnancy and postpartum there is an increased risk of developing mental disorders among women. Postpartum depression is the mental disorder that most affects mothers and has been considered a public health issue due to its high incidence, about 26% of mothers are affected. The arrival of the child requires rearrangements in the family structure. The transition from conjugality to parenting is the period of the family life cycle in which the investment oriented towards the husband-wife organization is transferred to the parent-child relationship. This change can generate conflicts that can compromise family bonds. This work aims to investigate the relationship between conjugality and the occurrence of postpartum depression in primiparous mothers. This research has a qualitative, descriptive and cross-sectional design of the case study type. The criteria for inclusion in the sample are: a) having gone through the birth process for at least 2 months and, at most, 36 months; b) be the only child, even if you have had previous pregnancies; c) having had a habitual risk pregnancy; d) baby without a diagnosis of syndromes or deformities; e) baby being born at term; f) be in a stable relationship.The instruments used are: Semi-open interview; Factorial Scale of Satisfaction with Couple Relationship (EFS-RC); sociodemographic questionnaire and Edinburgh Postpartum Depression Scale (EPDS). The results show that mental health is greatly impacted during the pregnancy-puerperal cycle with repercussions on the mother-baby bond and on the quality of the marital relationship. The five participants had more than one risk factor for postpartum depression. However, the only factor common to all is the weakened marital relationship. The pattern of marital-parental attachment in most cases is absent, where the father-husband is not a figure of support. In one case, the pattern is considered participatory, but with a result of marital satisfaction lower than all the previous ones. This is because the partner's participation is conditioned to be demanded by the mother-wife, which implies greater mental strain. Thus, the importance of conducting new studies on this theme is emphasized, as well as the need to carry out preventive intervention programs during pregnancy, with the objective of promoting health to women and couples, for the healthy experience of conjugality. and for the construction of engaged parenting, strengthening the family bond and for the reduction of the incidence of psychological disorders during the puerperium, especially postpartum depression.

A depressão pós parto é o transtorno mental que mais acomete puérperas e tem sido considerada uma questão de saúde pública em virtude de sua alta incidência, cerca de 26% das puérperas são acometidas. A chegada do filho exige rearranjos na estrutura familiar. A transição da conjugalidade para a parentalidade é o período do ciclo vital familiar em que o investimento orientado para a organização marido-mulher é transferido para a relação pais-filhos. Essa mudança pode gerar conflitos que podem comprometer os vínculos familiares. Este trabalho tem como objetivo investigar a relação entre a conjugalidade e a ocorrência de depressão pós-parto em puérperas primíparas. Essa pesquisa tem delineamento qualitativo, descritiva e transversal do tipo estudo de caso. Os critérios para a inclusão na amostra são: a) ter passado pelo processo de parto há, pelo menos, 2 meses e, no máximo, 36 meses; b) ser o único filho (a), mesmo que tenha tido gestações anteriores; c) ter tido uma gestação de risco habitual; d) bebê sem diagnóstico de síndromes ou deformidades; e) bebê ter nascido a termo e; f) estar num relacionamento estável. Os instrumentos utilizados são: Entrevista semi–aberta; Escala Fatorial de Satisfação com o Relacionamento de casal (EFS-RC); questionário sociodemográfico e Escala de Depressão Pós-Parto de Edimburgo (EPDS). Os resultados mostram que a saúde mental é muito impactada durante o ciclo gravídico-puerperal com repercussões na vinculação mãe-bebê e na qualidade do relacionamento conjugal. As cinco participantes apresentam mais de um fator de risco para depressão pós-parto. Porém, o único fator comum a todas é o relacionamento conjugal fragilizado. O padrão de vinculação conjugal-parental da maioria dos casos é o ausente, onde o pai-marido não é uma figura de apoio e suporte. Em um dos casos o padrão é considerado participativo, mas com um resultado de satisfação conjugal inferior a todos os anteriores. Isso se dá porque a participação do companheiro está condicionada a ser demandada pela mãe-esposa, o que implica em maior desgaste mental.
