Resumo: | This in vitro study evaluated the distribution of bending moments, compressive and tensile forces in abutments of Brånemark protocol prostheses, as a function of the inclination of the distal implants (straight or tilted) and the number of abutments (three, four, or five). Specimens were ten metallic bar structures cast in Pd-Ag alloy, simulating Brånemark protocol prostheses supported by five implants. These structures were made over two master models: one model with parallel and straight implants (n = 5) and one model with the distal implants tilted (n = 5). Strain gauges were fixed on the abutments of each master model to measure the deformation when a static load of 50 N was applied on the cantilever (15 mm) of each specimen. The deformation values were measured when the metallic bars were tested over three, four, or five abutments in the two master models. The deformation values were transformed into force and bending moment values and analyzed by ANOVA for randomized blocks design, followed by Tukey's test multiple comparisons, at the 5% level of significance. Abutment #1 (adjacent to cantilever under load) had the highest values of force and saggital bending moment for all tests with three, four, or five abutments. Independently from the number of abutments, the force in abutment #1 was higher in the straight model than in the tilted model. Total moment was higher with three abutments than with four or five abutments. Independently from the inclination of implants, the mean force with four or five abutments was lower than with three abutments. The results suggest that the inclination of posterior implants allow better distribution of axial forces and bending moments in abutments of Brånemark protocol prostheses supported by four or five abutments in comparison with the use of only three abutments. |

Foram fabricadas dez barras metálicas em liga de Prata-Paládio, as quais simularam uma prótese fixa tipo Protocolo Brånemark sobre dois modelos-mestre: um modelo com todos os implantes retos e paralelos entre si (n=5 barras) e outro com os dois implantes distais inclinados (n=5 barras). Extensômetros foram colados nos pilares protéticos de cada modelo-mestre para medir a deformação quando da aplicação de uma carga estática de 50 N no extremo livre (15 mm) de cada barra metálica. Os valores de deformação foram medidos quando as barras foram testadas sobre três, quatro e cinco pilares dos modelos reto e inclinado. Os valores de deformação dos pilares foram convertidos em força e momento fletor através de cálculos matemáticos e analisados por ANOVA para delineamento em blocos casualizados, complementada pelo Teste de Comparações Múltiplas de Tukey, ao nível de significância de 5%. O pilar 1 (adjacente ao cantilever sob carga) apresentou força e momento ântero-posterior significativamente maiores do que os demais pilares em todas as configurações. Para força no pilar 1, independentemente do número de pilares, o modelo reto apresentou maior força do que no modelo inclinado. O momento geral foi maior com três pilares em relação às configurações com quatro ou cinco pilares. Nas configurações com quatro e cinco pilares, independente da inclinação dos implantes, a média de força foi menor do que na configuração de três pilares. Os resultados sugerem que a inclinação dos implantes posteriores permite uma melhor distribuição de forças e momentos fletores em pilares para próteses tipo Protocolo Brånemark nas configurações com quatro e cinco pilares em relação à configuração com três pilares.Made available in DSpace on 2011-12-27T14:14:06Z (GMT).
