Análise das concentrações salivares de desidroepiandrosterona (DHEA) e de cortisol, escores de depressão, ansiedade e estresse em pacientes com líquen plano oral

Resumo: Lichen planus is a mucocutaneous disease, immunologicaly mediated by T lymphocytes, which manifests often in the oral mucosa. The mechanism that leads to the development of lesions remains unknown, but there is evidence suggesting the participation of psychological disturbances in the etiopathogenicity of this disease. The aim of this study was to identify the risk factors and salivary biomarkers for oral lichen planus (OLP). An analysis was made of the levels of dehydroepiandrosterone (DHEA) and cortisol, as well as the scores of depression, anxiety and stress in patients with this disease. Thirty-one patients with a histopathologic diagnosis of OLP were selected; they were of both sexes, with ages between 32 and 71 years. These patients were matched by sex and age with 31 control patients who did not present oral lesions. Symptoms of depression, anxiety and stress were investigated by the instruments The Beck Depression Inventory, The Beck Anxiety Inventory and Lipp’s Inventory of Symptoms of Stress for Adults, respectively. Saliva was collected in the morning and at night for determination of the DHEA and cortisol levels by radioimmunoassay. There was no significant difference between the groups with respect to scores of depression (P=0. 832), anxiety (P=0. 061) or stress (P=0. 611), or with respect to morning and night salivary levels of DHEA (P=0. 888, P=0. 297) and cortisol (P=0. 443, P=0. 983). Also, there was no difference between the groups with regard to the cortisol/DHEA ratio in the morning (P=0. 526) and night (P=0. 678) salivary samples. No correlation was observed between the salivary DHEA or cortisol levels and the scores of depression, anxiety and stress in the patients with OLP. In the sample investigated, it can be concluded that there is no association between the psychological variables depression, anxiety and stress with OLP. The levels of cortisol and DHEA or the ratio of these two steroids does not differ between individuals with the disease and controls, and therefore, these parameters cannot be used as salivary biomarkers for OLP.

O mecanismo que desencadeia o desenvolvimento das lesões permanece desconhecido, mas evidências sugerem a participação de transtornos psicológicos na sua etiopatogenia. Com o objetivo de identificar fatores de risco e biomarcadores salivares para o líquen plano oral (LPO), nesse estudo foram analisados os níveis de desidroepiandrosterona (DHEA) e de cortisol, bem como os escores de depressão, ansiedade e estresse em pacientes com essa doença. Foram selecionados 31 pacientes com diagnóstico clínico e histopatológico de LPO, de ambos os sexos, com idades entre 32 e 71 anos e 31 pacientes-controle, sem lesões bucais, emparelhados por sexo e faixa etária. Os sintomas de depressão, ansiedade e estresse foram investigados pelos instrumentos The Beck Depression Inventory, The Beck Anxiety Inventory e Inventário de Sintomas de Stress para Adultos de Lipp, respectivamente. Amostras de saliva foram coletadas pela manhã e à noite para análise das concentrações de DHEA e de cortisol, que foi realizada pela técnica de radioimunoensaio. Não houve diferença significativa entre os grupos quanto aos escores de depressão (p=0,832), ansiedade (p=0,061) ou estresse (p=0,611), nem quanto às concentrações salivares matinais e noturnas de DHEA (p=0,888, p=0,297) ou de cortisol (p=0,443, p=0,983). Também não houve diferença entre os grupos para o ratio cortisol/DHEA nas amostras salivares matinais (p=0,526) e noturnas (p=0,678). Não foi observada correlação entre as concentrações salivares de DHEA e de cortisol com os escores de depressão, ansiedade e estresse nos pacientes com LPO. Na amostra investigada pode-se concluir que não há associação entre as variáveis psicológicas depressão, ansiedade e estresse com o LPO.
