Resumo: | Introduction: The purpose of this study was to evaluate the immediate effects of the rapid maxillary expansion (RME) on the bone thickness and level of maxillary first molars, comparing the tooth-tissue-borne and tooth-borne expanders, in CBCT. Methodology: Two methodologies for periodontal evaluation were tested, one positioning the tomographic cut parallel to the palatal plane (A), and another positioning the cut parallel to the long axis of the dental root (B). After determining the best methodology, it was applied. The sample comprised of 66 CBCT exams (before expansion and immediate after expansion) from 33 patients. Multiplanar reconstruction was used to measure the following changes: buccal marginal bone level and thickness, interdental distance and intermolar angle. The Wilcoxon test and a mixed analysis of variance (ANOVA) model with alleatory effect to patients at a significance level of 5% were performed. Results: A mean reduction of 0,39 mm was observed on buccal bone thickness and a reduction of the buccal bone level of 0,55 mm was found as immediate effects after RME. Conclusions: The B methodology showed more reliable results for measuring the effects of RME on the buccal bone. The immediate effects of RME showed reduction of the buccal bone level and thickness of maxillary molars in both groups; the tooth-tissue-borne expander produced greater buccal crown tipping than did the tooth-borne expander; the intermolar distances increased in both groups, but the intermolar distance at the furcation level had a greater increase in the tooth-bourne group. |

Metodologia: Foram testadas duas metodologias para avaliação periodontal, uma posicionando os cortes tomográficos em relação ao plano palatal (A) e outra em relação ao longo eixo dentário (B). Após determinada a melhor metodologia, a mesma foi aplicada. Foram avaliadas 66 tomografias de 33 pacientes (pré-expansão e imediatamente após a expansão) em região de molares superiores, objetivando observar as seguintes alterações estruturais: altura e espessura óssea alveolar, alterações transversais e angulação intermolares. Foram realizados os testes estatísticos de Wilcoxon e de modelo misto para medidas repetidas, com efeito aleatório para paciente e intervalos de confiança em 95%.Resultados: Foi observada redução média na espessura óssea alveolar de 0,39mm e perda óssea vertical média de 0,55mm na região vestibular de molares superiores. Conclusão: a metodologia B mostrou-se mais adequada para avaliar os efeitos imediatos da ERM no periodonto de suporte, que foram: redução da altura e espessura óssea alveolar na vestibular dos molares superiores; inclinação dos molares para vestibular em ambos os grupos, sendo significativamente maior no Grupo Haas; aumento das dimensões transversais intermolares em ambos os grupos, sendo que a distância intermolares ao nível de furca aumentou significativamente mais no Grupo Hyrax.Made available in DSpace on 2011-12-27T14:14:06Z (GMT).
