Eosinophilic esophagitis in the child [Abstract in English]

Resumo: Aims: To offer to the pediatrician scientific information for clinical suspicion, early diagnosis and up to date in the management of eosinophilic esophagitis. Source of data: The information was collected from Medline. The authors reviewed the most relevant national and international medicine papers in the last 20 years. Summary of the findings: The following topics are discussed: incidence, physiopathology, genetics, symptomatology, diagnostic and treatment, with special emphasis to the treatment (dietetic, medications and endoscopic treatment). Conclusions: The eosinophilic esophagitis is an isolated eosinophilic inflammation of the esophagus in which the symptoms are mistakenly understood to be the ones produced by the gastroesophangeal reflux illness. The incidence of this pathology/illness has increased lately, being diagnosed by means of upper digestive endoscopy and biopsy procedures. The treatment consists of a diet of allergen release as well as the use of local or systemic corticosteroids. KEY WORDS: ESOPHAGITIS; ESOPHAGEAL DISEASES; EOSINOPHILS; SIGNS AND SYMPTOMS, DIGESTIVE.

Fonte de dados: Foram selecionados, através do MEDLINE, os artigos de periódicos médicos nacionais e internacionais mais relevantes sobre o tema dos últimos 20 anos. Síntese dos dados: São abordados os seguintes tópicos: incidência, etiologia, fisiopatologia, genética, sintomatologia, diagnóstico e tratamento, com ênfase especial no tratamento de eliminação, medicamentoso e endoscópico. Conclusões: Esofagite eosinofilica é uma inflamação com infiltrado de eosinófilos, que ocorre de forma isolada no esôfago, onde os sintomas, usualmente, são confundidos com os da doença por refluxo gastresofágico. A incidência desta doença vem aumentando nos últimos anos, sendo diagnosticada através da endoscopia digestiva alta e biópsias. O tratamento consiste em uma dieta de eliminação dos alérgenos envolvidos, assim como corticosteróides tópicos ou sistêmicos.
