Implicações no seguimento das orientações fonoaudiológicas com relação aos quadros disfágicos pediátricos

Resumo: | INTRODUCTION: In cases of pediatric dysphagia, tube feeding is a complex issue for patients and their families. Despite the orientations received, families often transgress the proposed treatment by offering foods orally, which can result in clinical complications. PURPOSE: To identify and describe the conditions that imply in adherence the speech therapy orientation regarding pediatric dysphagia. METHOD: exploratory, descriptive study. Sample: 40 speech therapists working in pediatric dysphagia and 10 mothers of children between 2 and 19 years old with dysphagia and using tube feeding. Procedures: study developed in two complementary parts. Part 1 - applied an online questionnaire with speech therapists. Part 2 - semi-structured interview, recorded and transcribed, with the mothers. Analysis of the results: the data of the questionnaire with speech therapists were tabulated and analyzed descriptively; after that, discourse analysis was performed from the transcript of interviews with family members. RESULTS: 85% of professionals treated children aged 0 to 5 years and 15% between 5 and 10 years old; 67.5% of speech therapists declared occurrence of transgressions during rehabilitation process; and in the face of episodes of transgression, 88.9% reorient the families about the child's clinical condition. Analyzing maternal speeches, mothers related the occurrence of transgressions, justified by the pleasure of oral feeding and the desire for feed their child. CONCLUSION: after data analysis it was possible to identify the occurrence of transgressions to the proposed treatment. In that cases, the conduct of professionals included, mainly, reorientation about the risks of bronchoaspiration and necessary care. In the maternal speeches, the understanding of the child's clinical condition was identified; however, there is a desire to feed and to exercise mothering. Thus, addressing mothering in pediatric swallowing rehabilitation and elaborate a Singular Therapeutic Project seem fundamental to ensure the success of treatment |

Apesar das orientações recebidas, muitas vezes, as famílias transgridem o tratamento proposto ofertando alimentos por via oral, o que pode resultar em complicações clínicas. OBJETIVO: identificar e descrever as condições que implicam no seguimento das orientações fonoaudiológicas com relação aos quadros disfágicos pediátricos. MÉTODO: estudo exploratório, descritivo. Casuística: 40 fonoaudiólogos que atuavam em disfagia pediátrica e 10 mães de crianças com idade entre 2 e 19 anos, diagnóstico de disfagia e uso de via alternativa de alimentação. Procedimentos: estudo desenvolvido em duas partes complementares. Parte 1 – aplicado um questionário online com os fonoaudiólogos. Parte 2 – realizada entrevista semiestruturada com as mães, a entrevista foi gravada e transcrita. Análise dos dados: os resultados do questionário com fonoaudiólogos foram tabulados e analisados de forma descritiva; em seguida, realizada análise do discurso a partir da transcrição das entrevistas com familiares. RESULTADOS: 85% dos profissionais atendiam crianças com idade entre 0 a 5 anos e 15% entre 5 a 10 anos de idade; 67,5% dos fonoaudiólogos afirmaram a ocorrência de transgressões durante o processo de reabilitação; e diante dos episódios de transgressão, 88,9% reorientou as famílias acerca do quadro clínico da criança. Sobre o discurso materno, as mães relataram a ocorrência das transgressões, justificadas no prazer da alimentação por via oral e no desejo de alimentar. CONCLUSÃO: após a análise dos dados foi possível identificar a ocorrência de transgressões ao tratamento proposto. Diante desses episódios, as condutas dos profissionais incluem, principalmente, a reorientação sobre os riscos de broncoaspiração e cuidados necessários. Nas falas maternas, identificou-se a compreensão do quadro clínico da criança, entretanto, há um desejo de alimentar e de exercer maternagem.
