Avaliação do tônus muscular com o uso do aplicativo angle meter

Resumo: Introduction: Non-evolutionary encephalopathy, or cerebral palsy, involves deficits in body structures and functions. It is associated with loss of motor function, due to spasticity, with increased muscle tone. Muscle tone is assessed using the Ashworth and Tardieu scales, applying passive stretching to a muscle group at two different speeds (fast and slow) and measuring the angle of resistance to passive stretching. Objective: This study aimed to describe the psychometric characteristics of the Tardieu scale for assessing muscle tone (mean, standard deviation, and variance). Method: This is an observational cross-sectional study, which analyzed 15 evaluations, carried out by three physiotherapists of the muscle tone of a left spastic hemiparetic patient. Through videos and the angle meter application, muscle tone was assessed, according to the criteria of the Ashworth and Tardieu scales. Result: In measuring the angles of the "M" evaluator, there was a greater perception of muscle resistance to passive mobilization. The "L" evaluator presented a lower perception of resistance to passive mobilization. In the measurements of the "G" and "M" evaluators, there was a greater perception of resistance (V3) in the hip flexor muscles. However, V3 was at most 5°, indicating a small change in joint angle during fast and slow mobilizations. The data from the ‘‘L’ and ‘M’’ evaluators showed low variance of V3 of the knee flexors, when compared with the results of the ‘‘G’ evaluator. In the measurements of the "L" evaluator, there were greater knee flexion angles at fast speed. The ''L" and "M'' evaluators showed greater variability of V3, compared with the results of the ''G" evaluator. Conclusion: The Tardieu scale showed low variance and small standard deviation, with good psychometric characteristics in measuring muscle tone. The initial angle at slow speed was difficult to identify, due to the low degree of spasticity. The assessment of the joint angle using the angle meter is an effective and complementary way to understand muscle tone. The Tardieu scale, associated with the angle meter application, is feasible for measuring spasticity, as it evaluates and compares the muscle response to passive mobilization at both speeds

O tônus muscular é avaliado pelas escalas de Ashworth e Tardieu, com a aplicação de alongamento passivo a um grupo muscular em duas velocidades diferentes (rápida e lenta) e pela medida do ângulo de resistência ao alongamento passivo. Objetivo: O presente estudo visou descrever características psicométricas da escala de Tardieu para avaliação do tônus muscular (média, desvio-padrão e variância). Método: Trata-se de um estudo transversal observacional, que analisou 15 avaliações, realizadas por três fisioterapeutas, do tônus muscular de um paciente hemiparético espástico à esquerda. Por meio de vídeos e do aplicativo angle meter, o tônus muscular foi avaliado, de acordo com os critérios das escalas de Ashworth e Tardieu. Resultado: Na medida dos ângulos da avaliadora "M", houve maior percepção de resistência do músculo à mobilização passiva. O avaliador "L" apresentou menor percepção de resistência à mobilização passiva. Nas medidas dos avaliadores "G” e “M", houve maior percepção de resistência (V3) nos músculos flexores de quadril. No entanto, V3 foi de, no máximo, 5°, indicando pequena mudança de ângulo articular durante as mobilizações rápida e lenta. Os dados dos avaliadores ‘‘L” e “M’’ apresentaram baixa variância de V3 dos flexores de joelho, quando comparados com os resultados do avaliador ‘‘G”. Nas medidas do avaliador "L", houve maiores ângulos de flexão de joelho na velocidade rápida. Os avaliadores ''L” e “M'' mostraram maior variabilidade de V3, comparados com os resultados do avaliador ''G". Conclusão: A escala de Tardieu apresentou baixa variância e pequeno desvio-padrão, com boa característica psicométrica na mensuração do tônus muscular. O ângulo inicial na velocidade lenta foi difícil de identificar, devido ao baixo grau de espasticidade. A avaliação do ângulo articular pelo angle meter é uma forma eficaz e complementar para compreender o tônus muscular.
