Resumo: | This is a doctoral thesis that was carried out in the Study Group on Career Development (GEDC) of the Pontifical Catholic University of Rio Grande do Sul. The research problem to be investigated arose from the concern about the possibility of Burnout being a variable involved on the intention of leaving the job and on the Turnover of adult professionals. In view of my professional career also in the field of nursing, the construction of the doctoral project was concentrated in the nursing population, for which Burnout is very present. In order to get a closer look at the relationship between the Burnout variables and the Turnover intention, a Systematic Review (RS) was carried out, through which it was possible to verify the strong relationship between Burnout and intention to quit. This SR, titled "The Predictive Function of Burnout Syndrome for the Turnover in Nursing Professionals: Systematic Review" was submitted and published in Volume 3 of 2017 of the journal "Advances in Latin American Psychology". The SR also allowed us to conclude that the research problem was coherent and pertinent to the Brazilian field of research, since no work was found on the topic of the article that had been produced in the national context. Another important result indicated the global trend of a decrease in the number of nursing professionals, to the point of speaking about the shortage of nursing workers, both in Western and Eastern countries. This fact reinforced the proposal to investigate possible predictors of the increase of the voluntary Turnover in nursing. It was necessary to define the instruments that would be used for the measurement of burnout and the intention of Turnover, justifying the relevance of the study of prediction analysis of Burnout on Turnover Intent. This fact, added to the objective of using instruments developed specifically for the Brazilian context, led to the choice of an instrument developed by Tamayo and Tróccoly (2009) to evaluate Burnout through three dimensions: emotional exhaustion, dehumanization and disappointment at work. The use of the term dehumanization as a substitute for depersonalization (used in the Maslach Scale) is adequate, not only because it is a more proper use of nursing, but because it is a concern of the entire Brazilian public health system, to which humanization has become a goal to be achieved in patient care. Following the doctoral proposal to develop and make available new research tools for psychology, the scale for the evaluation of Turnover's intention was adapted from an initial proposal developed by Hom and Griffeth (1991). The choice of this instrument is due to the fact that it encompasses two factors considered in the literature that are important for the study of the intention of Turnover, namely the cognitions of exit from employment and the search behavior of employment, and therefore, an innovative instrument. The study of cultural adaptation and validation of the instrument for use in the Brazilian context has been submitted and accepted for publication in the journal Psychological Evaluation, in a special edition consisting of studies that involved innovative methods of analysis. In the case of this article: "Psychometric properties of the Turnover Intentionality Scale and Job Search (EITBE)" the analyzes were carried out with Programming R. In the research sequence, we sought to have a cohesive study line from the beginning to the end of the doctorate degree. In this sense, the research project was built with the aim of investigating Burnout and its direct relation with the intention of Turnover in the nursing population and the doctoral thesis was constituted in the defense that the Burnout was an important predictor for the intention of Turnover. From the SR, career theoretical references and organizational psychology, the research was proposed to investigate variables with possible relationships with Burnout and Turnover intent, which encompassed personal, organizational and contextual factors. They entered the study as independent variables: career adaptability, organizational commitment, Coping strategies and employment opportunity variables. In the first moment these VIs and their predictive potential with respect to the Burnout were tested and in the second moment these VIs were tested with the addition of Burnout related to the intention of Turnover. These tests consisted of multiple linear regression analyzes and resulted in two articles, article 3: "Predicting variables for Burnout syndrome among nursing professionals" a prediction analysis for Burnout and article 4: "Predictors variables for the Intention of Turnover among Nursing Professionals "prediction analysis for Turnover intent, using the EITBE scale. Brief description of the studies. In study 1, a systematic review (SR) about Burnout's prediction of the intention to quit. Study 2 was a validation and cultural adaptation of EITBE (Turnover Intention Scale and Job Search). From the cultural adaptation, the instrument was validated through an evaluation of the factorial structure of the scale (Exploratory Factor Analysis (AFE) and Confirmatory Factor Analysis (AFC) through Program R. The statistical analysis showed adequate adjustment factors for its In order to produce a suitable and sufficient model for a factorial analysis, 5 to 20 participants per item of the evaluated instrument were required, due to the heterogeneous composition of the sample. The sample was made up of 201 professionals from various areas of the southern region of Brazil and the data were stored on the Qualtrics platform. Study 3 had the objective of the study was to analyze how occupational impairment and perceived job opportunities (organizational factors), career adaptability and Coping (individual factors) contribute to the prediction of Burnout. This quantitative study had a sample of 246 nurses from three hospitals in the southern region of Brazil. Data were subjected to stepwise backward multiple regressions using SPSS software. Predictive analysis of all independent variables with respect to Burnout characterization revealed a R²aj (predictive factor) of 49.5%. The results confirm that the organizational factors trigger the Burnout, although, among the results, it is possible to perceive also the importance of individual factors to trigger the Burnout syndrome. It is possible to highlight the need to build an environment that minimizes the predictive variables of Burnout. In study 4 we used the same explanatory variables added from the Burnout to analyze the best predictors for the intention of Turnover using the EITBE scale. The aim of this study was to analyze the predictive factor about the Turnover intention of variables present in the nursing work environment. The research sample consisted of 246 nursing professionals from three hospitals in the southern region of Brazil. The quantitative study was analyzed through multiple linear regression using SPSS software version 22. The results indicate that Turnover's intention is a result of personal, organizational and contextual factors. Organizational commitment and Burnout presented the major explanatory factors for Turnover's intent. There is a need to adapt the work environment, valuation of the nursing professional and prevention of ill-health factors, which can prevent suffering and encourage the maintenance of qualified human capital. |

A ideia inicial de estudo surgiu da inquietação sobre a possibilidade de o Burnout ser uma variável envolvida na intenção de saída do emprego e no Turnover. Tendo em vista a minha trajetória profissional também no campo da enfermagem, a construção do projeto de doutorado foi concentrada na população de enfermagem em que o Burnout está bastante presente. Para ter uma noção do campo de pesquisa sobre as variáveis Burnout e intenção de Turnover, foi realizado uma Revisão Sistemática (RS) em que se pôde perceber a forte relação entre Burnout e intenção de sair do emprego. Esta RS virou um artigo com o título: “A função preditora da síndrome de Burnout para o Turnover nos profissionais de enfermagem: revisão sistemática” que foi submetido na revista “Avances em psicologia latinoamericana” em 2015 e publicado no volume 3 de 2017. A RS foi importante para mostrar que a ideia da tese era coerente e pertinente ao campo de pesquisa brasileiro, uma vez que não foi encontrado nenhum trabalho na temática do artigo que tivesse sido produzido no Brasil. Um resultado importante mostrado pelo artigo foi uma tendência global de diminuição dos profissionais atuantes em enfermagem a ponto de se falar em escassez de trabalhadores de enfermagem, tanto em paises ocidentais e orientais. Este fato reforçou a proposta de se estudar possíveis variáveis que podem estar por trás do aumento do Turnover voluntário em profissionais da enfermagem. Justificado o estudo de análise de predição do Burnout sobre Intenção de Turnover, foi preciso definir os instrumentos que seriam utilizados para a medição do Burnout e intenção de Turnover. Além disto, pensou-se em investir em alguma escala de autores brasileiros que tivessem estudado especificamente a população de enfermagem. A escala escolhida de autoria de Tamayo e Tróccoly (2009) avalia o Burnout em três dimensões que são: exaustão emocional, desumanização e decepção no trabalho. O uso do termo desumanização ao invéz de despersonalização é adequado não só por ser de uso mais próprio da enfermagem, mas por ser uma preocupação de todo o sistema saúde público brasileiro em que a humanização passou a ser uma meta a ser alcançada no atendimento aos pacientes. Seguindo a proposta do doutorado de desenvolver e disponibilizar novos instrumentos de pesquisa para a psicologia, a escala para a avaliação da intenção de Turnover foi adaptada a partir de uma proposta inicial desenvolvida por Hom e Griffeth (1991). A escolha deste instrumento se deve ao fato de que ele engloba dois fatores considerados pela literatura importantes para o estudo da intenção de Turnover, a saber as cognições de saída do emprego e o comportamento de busca de emprego, sendo portanto, um instrumento inovador. O estudo de adaptação cultural e validação do instrumento para uso no contexto brasileiro encontra-se submetido e foi aceito para publicação na Revista Avaliação psicológica, em uma edição especial constituída por estudos que envolveram métodos inovadores de análise. No caso deste artigo: “Propriedades psicométricas da Escala de Intenção de Turnover e Busca de Emprego (EITBE)” as análises foram realizadas com a Programação R. Na sequência de pesquisa, buscou-se ter uma linha de estudo coesa do início ao fim do doutorado. Neste sentido se construiu o projeto de pesquisa com a meta de investigar o Burnout e sua relação direta com a intenção de Turnover na população de enfermagem e a tese de doutorado se constituiu na defesa de que o Burnout fosse um importante preditor para a intenção de Turnover. A partir da RS, referenciais teóricos de carreira e psicologia organizacional se propôs a investigação de variáveis com possiveis relações com Burnout e com intenção de Turnover, que englobasse fatores pessoais, organizacionais e contextuais. Entraram no estudo como variáveis independentes: adaptabilidade de carreira, comprometimento organizacional, estratégias de Coping e variáveis de oportunidade de emprego. Num primeiro momento foram testados estas VIs e o seu potencial preditor com relação ao Burnout e num segundo momento foram testados estas VIs com a adição do Burnout relacionados à intenção de Turnover. Estas testagens 13 constituíram de análises de regressão linear múltipla e resultaram em dois artigos, sendo o artigo 3: “Variáveis preditoras para a Síndrome de Burnout entre profissionais de enfermagem” uma análise de predição para o Burnout e o artigo 4: “Variáveis preditoras para a Intenção de Turnover entre profissionais de enfermagem” a análise de predição para a intenção de Turnover. Breve descrição dos estudos da tese O estudo teve como objetivo geral: investigar as relações entre a adaptabilidade de carreira, as estratégias de Coping, o comprometimento organizacional, índices de oportunidade de emprego, Burnout e intenção de Turnover e busca de emprego entre profissionais de enfermagem. Foi adotado um delineamento quantitativo dividido em 4 estudos. No estudo 1 foi realizada uma revisão sistemática (RS) sobre a predição do Burnout com relação à intenção de sair do emprego. O objetivo do estudo foi investigar as relações entre a Síndrome de Burnout, comportamento e intenção de Turnover em equipes de enfermagem por meio de uma revisão sistemática da literatura nacional e internacional. Método: Seguiram-se os passos de pesquisa sugeridos pelo método Cochrane. A pesquisa foi realizada nos meses de fevereiro a abril de 2015 nas bases de dados BDENF, MEDLINE, LILACS, PEPSIC, SCOPUS e Periódicos CAPES. Foram incluídos apenas artigos completos, que guardavam relação direta com a pergunta de pesquisa, publicados entre o período de janeiro de 2010 a março de 2015, resultando em 14 artigos. Resultados: observou-se relação positiva e preditora do Burnout sobre o Turnover nos profissionais de enfermagem. Evidencia-se que as consequências do Burnout e do Turnover recaem sobre o sujeito adoecido, a equipe que permanece no emprego e também sobre a organização hospitalar. A relação Burnout-Turnover entre profissionais de enfermagem é uma questão séria e que precisa ser tratada preventivamente com ações de formação e atenção aos profissionais de enfermagem. 14 O estudo 2 foi uma validação e adaptação cultural da EITBE (Escala de Intenção de Turnover e Busca de Emprego). A partir da adaptação cultural foi realizada a validação do instrumento através de uma avaliação da estrutura fatorial da escala (Análise Fatorial Exploratória (AFE) e Análise Fatorial Confirmatória (AFC) por meio do Programa R. A análise estatística mostrou fatores de ajustes adequados para seu uso em pesquisa. A composição da amostra seguiu as orientações de Byrne (2010), de que para produzir um modelo adequado e suficiente para uma análise fatorial são necessários 5 a 20 participantes por item do instrumento avaliado. Devido à composição heterogênea da amostra, foram considerados 20 participantes por item do instrumento, totalizando 200 participantes. A amostra foi de 201 profissionais de diversas áreas da região sul do Brasil e os dados foram armazenados na plataforma Qualtrics. No estudo 3 e 4 a população de estudo foi profissionais de enfermagem da região de Porto Alegre e Santa Maria. A amostra de pesquisa foi de 246 profissionais, incluindo técnicos de enfermagem e enfermeiros. O estudo 3 teve o objetivo do estudo foi analisar como o comprometimento ocupacional e oportunidades de trabalho percebidas (fatores organizacionais), adaptabilidade de carreira e Coping (fatores individuais) contribuem para a predição de Burnout. Este estudo quantitativo contou com uma amostra de 246 enfermeiros de três hospitais da região sul do Brasil. Os dados foram submetidos a regressões múltiplas stepwise backward utilizando o software SPSS 22. Análise preditiva de todas as variáveis independentes com relação à caracterização de Burnout revelou um R²aj (fator preditivo) de 49,5%. Os resultados confirmam que os fatores organizacionais desencadeiam o Burnout, embora, entre os resultados, seja possível perceber também a importância de fatores individuais desencadearem a síndrome de Burnout. É possível destacar a necessidade de construir um ambiente que minimize as variáveis preditivas de Burnout. No estudo 4 utilizou-se as mesmas variáveis explicativas acrescentadas do Burnout para analisar os melhores preditores para a intenção de Turnover usando a escala EITBE. 15 Objetivou-se analisar o fator preditivo sobre a intenção de Turnover de variáveis presentes no ambiente de trabalho da enfermagem. A amostra de pesquisa foi de 246 profissionais de enfermagem de três hospitais da região sul do Brasil. O estudo de caráter quantitativo foi analisado por meio de regressão linear múltipla através do software SPSS versão 22. Os resultados indicam que a intenção de Turnover é resultante de fatores pessoais, organizacionais e contextuais. O comprometimento organizacional e o Burnout apresentaram os maiores fatores de explicação para a intenção de Turnover. This is a doctoral thesis that was carried out in the Study Group on Career Development (GEDC) of the Pontifical Catholic University of Rio Grande do Sul. The research problem to be investigated arose from the concern about the possibility of Burnout being a variable involved on the intention of leaving the job and on the Turnover of adult professionals. In view of my professional career also in the field of nursing, the construction of the doctoral project was concentrated in the nursing population, for which Burnout is very present. In order to get a closer look at the relationship between the Burnout variables and the Turnover intention, a Systematic Review (RS) was carried out, through which it was possible to verify the strong relationship between Burnout and intention to quit. This SR, titled "The Predictive Function of Burnout Syndrome for the Turnover in Nursing Professionals: Systematic Review" was submitted and published in Volume 3 of 2017 of the journal "Advances in Latin American Psychology". The SR also allowed us to conclude that the research problem was coherent and pertinent to the Brazilian field of research, since no work was found on the topic of the article that had been produced in the national context. Another important result indicated the global trend of a decrease in the number of nursing professionals, to the point of speaking about the shortage of nursing workers, both in Western and Eastern countries. This fact reinforced the proposal to investigate possible predictors of the increase of the voluntary Turnover in nursing. It was necessary to define the instruments that would be used for the measurement of burnout and the intention of Turnover, justifying the relevance of the study of prediction analysis of Burnout on Turnover Intent. This fact, added to the objective of using instruments developed specifically for the Brazilian context, led to the choice of an instrument developed by Tamayo and Tróccoly (2009) to evaluate Burnout through three dimensions: emotional exhaustion, dehumanization and disappointment at work. The use of the term dehumanization as a substitute for depersonalization (used in the Maslach Scale) is adequate, not only because it is a more proper use of nursing, but because it is a concern of the entire Brazilian public health system, to which humanization has become a goal to be achieved in patient care. Following the doctoral proposal to develop and make available new research tools for psychology, the scale for the evaluation of Turnover's intention was adapted from an initial proposal developed by Hom and Griffeth (1991). The choice of this instrument is due to the fact that it encompasses two factors considered in the literature that are important for the study of the intention of Turnover, namely the cognitions of exit from employment and the search behavior of employment, and therefore, an innovative instrument. The study of cultural adaptation and validation of the instrument for use in the Brazilian context has been submitted and accepted for publication in the journal Psychological Evaluation, in a special edition consisting of studies that involved innovative methods of analysis. In the case of this article: "Psychometric properties of the Turnover Intentionality Scale and Job Search (EITBE)" the analyzes were carried out with Programming R. In the research sequence, we sought to have a cohesive study line from the beginning to the end of the doctorate degree. In this sense, the research project was built with the aim of investigating Burnout and its direct relation with the intention of Turnover in the nursing population and the doctoral thesis was constituted in the defense that the Burnout was an important predictor for the intention of Turnover. From the SR, career theoretical references and organizational psychology, the research was proposed to investigate variables with possible relationships with Burnout and Turnover intent, which encompassed personal, organizational and contextual factors. They entered the study as independent variables: career adaptability, organizational commitment, Coping strategies and employment opportunity variables. In the first moment these VIs and their predictive potential with respect to the Burnout were tested and in the second moment these VIs were tested with the addition of Burnout related to the intention of Turnover. These tests consisted of multiple linear regression analyzes and resulted in two articles, article 3: "Predicting variables for Burnout syndrome among nursing professionals" a prediction analysis for Burnout and article 4: "Predictors variables for the Intention of Turnover among Nursing Professionals "prediction analysis for Turnover intent, using the EITBE scale. Brief description of the studies. In study 1, a systematic review (SR) about Burnout's prediction of the intention to quit. Study 2 was a validation and cultural adaptation of EITBE (Turnover Intention Scale and Job Search). From the cultural adaptation, the instrument was validated through an evaluation of the factorial structure of the scale (Exploratory Factor Analysis (AFE) and Confirmatory Factor Analysis (AFC) through Program R. The statistical analysis showed adequate adjustment factors for its In order to produce a suitable and sufficient model for a factorial analysis, 5 to 20 participants per item of the evaluated instrument were required, due to the heterogeneous composition of the sample. The sample was made up of 201 professionals from various areas of the southern region of Brazil and the data were stored on the Qualtrics platform. Study 3 had the objective of the study was to analyze how occupational impairment and perceived job opportunities (organizational factors), career adaptability and Coping (individual factors) contribute to the prediction of Burnout. This quantitative study had a sample of 246 nurses from three hospitals in the southern region of Brazil. Data were subjected to stepwise backward multiple regressions using SPSS software. Predictive analysis of all independent variables with respect to Burnout characterization revealed a R²aj (predictive factor) of 49.5%. The results confirm that the organizational factors trigger the Burnout, although, among the results, it is possible to perceive also the importance of individual factors to trigger the Burnout syndrome. It is possible to highlight the need to build an environment that minimizes the predictive variables of Burnout. In study 4 we used the same explanatory variables added from the Burnout to analyze the best predictors for the intention of Turnover using the EITBE scale. The aim of this study was to analyze the predictive factor about the Turnover intention of variables present in the nursing work environment. The research sample consisted of 246 nursing professionals from three hospitals in the southern region of Brazil. The quantitative study was analyzed through multiple linear regression using SPSS software version 22. The results indicate that Turnover's intention is a result of personal, organizational and contextual factors. Organizational commitment and Burnout presented the major explanatory factors for Turnover's intent. There is a need to adapt the work environment, valuation of the nursing professional and prevention of ill-health factors, which can prevent suffering and encourage the maintenance of qualified human capital. |
