Resumo: | Squamous cell carcinoma (SCC) is the most prevalent malignant neoplasm of the oral cavity, with significant rates of morbidity and mortality. The role of inflammation in tumor initiation, growth, and progression, as well as in the survival of patients with oral cancer is still unclear. Given the large number of researches evaluating the relationship of inflammatory markers with oral cancer and the heterogeneity of results, we developed a systematic review and meta-analysis to investigate the evidence regarding the association of the tumor necrosis factor α (TNF-α), toll-like receptor 4 (TLR4), I𝜅B kinase α (IKKα) and cyclo-oxygenase 2 (COX-2) with the survival of patients with oral SCC (OSCC). A systematic review was conducted in the electronic databases of PubMed, Web of Science, LILACS, EMBASE, Scopus, and Cochrane Library by two independent researchers. The PECO question was “Does the overexpression of tumor necrosis factor α, cyclooxygenase 2, I𝜅B α kinase or toll-like Receptor 4 influence the survival of patients with OSCC?”. Prospective and/or retrospective observational studies that evaluated the association of the expression of the cited markers with the outcomes of overall survival, disease-specific survival, disease-free survival, survival probability, and 5-year survival were selected. Twentyone articles met the eligibility criteria for inclusion in the systematic review, as being fifteen on COX-2, three on TLR4, two on IKKα, and one about TNF-α. Three studies that address the COX-2 marker presented sufficient data to develop a meta-analysis. The estimated hazard ratio (HR) was 1.51 (95% CI 0.97, 2.33) for the association of COX-2 overexpression with the overall survival outcome, however, this was not statistically significant (p=0.07). O papel da inflamação na iniciação, crescimento e progressão tumorais, assim como na sobrevida de pacientes com câncer de boca ainda não está esclarecido. Tendo em vista o grande número de pesquisas estudando a relação de marcadores inflamatórios com o câncer de boca e a heterogeneidade de resultados, neste estudo foi realizada uma revisão sistemática e meta-análise com o objetivo de investigar as evidências quanto à associação do fator de necrose tumoral α (TNF-α), receptor toll-like 4 (TLR4), quinase I𝜅B α (IKKα) e ciclooxigenase 2 (COX-2) com a sobrevida de pacientes com CEC bucal. Uma revisão sistemática foi conduzida nas bases eletrônicas de dados PubMed, Web of Science, LILACS, EMBASE, Scopus e Cochrane Library por dois pesquisadores independentes. A pergunta “a superexpressão do fator de necrose tumoral α, da ciclooxigenase 2, da quinase I𝜅B α ou do receptor toll-like 4 exerce influência na sobrevida de pacientes com CEC de boca?” foi estabelecida como a questão PECO. Estudos observacionais prospectivos e/ou retrospectivos, que avaliaram a associação da expressão dos marcadores citados com os desfechos de sobrevida geral, sobrevida específica da doença, sobrevida livre de doença, probabilidade de sobrevida e sobrevida em 5 anos foram selecionados. Vinte e um artigos preencharam os critérios de eligibilidade para inclusão na revisão sistemática, sendo quinze sobre a COX-2, três sobre o TLR4, dois sobre o IKKα e um sobre o TNF- α. Três estudos que abordam o marcador COX-2 apresentaram dados suficientes para o desenvolvimento de uma meta-análise. O valor de hazard ratio (HR) estimado foi de 1.51 (IC 95% 0.97, 2.33) para associação da superexpressão da COX-2 com o desfecho de sobrevida geral, entretanto este não foi estatisticamente significativo (p=0,07). Uma baixa heterogeneidade foi observada entre os estudos avaliados na meta-análise (I2=0%). O número de estudos incluídos foi insuficiente para o desenvolvimento de meta-análises dos marcadores TNF-α, TLR4 e IKKα. Os resultados apontam para uma provável associação positiva entre a expressão dos marcadores citados e menores índices de sobrevida dos pacientes com CEC bucal.
