Healthcare Professionals Perception of the Reconciliation Medication Process in a Hospital Context: Case Study

Resumo: Introduction – The medication reconciliation (MR) process aims, in moments of transition between levels of healthcare provision, to reduce incidents with medication and contribute to the safe use of medication. This study aimed to explore barriers and factors that influence the MR process in an Internal Medicine service of a district level hospital and to identify opportunities for improvement. Methods – Case study of a qualitative nature, of single and exploratory type, with semi-structured interviews with physicians, nurses and pharmacists. Results – Difficulties in communication with the patient, family, among health professionals, services and other institutions stand out. At the Information Technology (IT) level, difficulties in prescribing in an emergency context, repeated medication alerts and non-access to the Plataforma de dados de Saúde (PDS) by pharmacists. As opportunities for improvement of MR, healthcare professionals pointed to training, patient follow-up after hospital discharge, improved communication between healthcare institutions and integration between healthcare information systems. Discussion /Conclusion – The obtained results demonstrate that barriers to the MR process persist, with expected consequences in terms of the quality of care provided. This study alerts to the need to implement measures to mitigate them.

This study alerts to the need to implement measures to mitigate them.Introdução – O processo de reconciliação terapêutica (RT) visa, em momentos de transição entre níveis de prestação de cuidados de saúde, diminuir os incidentes com medicamentos e contribuir para o uso seguro da medicação. Com este estudo pretendeu-se explorar barreiras e fatores que influenciam o processo de RT num serviço de Medicina Interna de um hospital de nível distrital e identificar oportunidades de melhoria. Métodos – Estudo de natureza qualitativa, do tipo de estudo de caso único e exploratório, com a realização de entrevistas semiestruturadas a médicos, enfermeiros e farmacêuticos. Resultados – Destacam-se as dificuldades na comunicação com o doente, família, entre os profissionais de saúde, serviços e outras instituições. A nível informático, dificuldades na prescrição em contexto de emergência, alertas de medicação repetidos e o não acesso à plataforma de dados de saúde (PDS) pelos farmacêuticos. Como oportunidades de melhoria na RT, os profissionais de saúde apontaram para a formação, seguimento do doente após alta hospitalar, melhoria da comunicação entre as instituições de saúde e integração entre os sistemas de informação em saúde. Discussão/Conclusões - Os resultados obtidos demonstram que as barreiras ao processo de RT persistem, com consequências expetáveis a nível da qualidade dos cuidados prestados.
