Porém, em certas hemoglobinopatias, o aumento do nível de HbF poderá assumir grande importância por melhorar o quadro clínico dos doentes, nomeadamente, nos portadores de anemia falciforme e de β-talassemia. A descoberta de que a regulação da HbF no organismo adulto está dependente de mecanismos genéticos abriu caminho para a investigação e identificação dos loci envolvidos no processo. Até ao presente diversos estudos de associação, realizados em diferentes amostras populacionais, identificaram três loci principais envolvidos na regulação da produção de HbF: o polimorfismo rs7482144, situado na região promotora do gene HBG2, o gene BCL11A e a região intergénica HMIP (HBS1L-MYB). Entretanto, outras investigações identificaram novos polimorfismos que poderão estar envolvidos na variação dos níveis de HbF, nomeadamente, LCR-5’HS4 rs16912979, KLF1 rs79334031, DCHS2 rs61746132, RNF113B rs16955011 e ARHGAP18 rs11759328. O objetivo principal da presente dissertação foi estudar diferentes loci candidatos a reguladores dos níveis de HbF numa amostra de doentes com β-talassemia minor, de nacionalidade portuguesa, e testar a sua associação com a variação dos níveis de HbF. Foram estudadas 66 amostras de indivíduos portugueses com β-talassemia minor, 30 do sexo masculino e 36 do sexo feminino, com idades entre 2 e 77 anos, diagnosticados no Serviço de Hematologia Clínica do Centro Hospitalar e Universitário de Coimbra (CHUC). Os indivíduos são saudáveis e assintomáticos, com valores de HbF entre 0,2% e 9% e níveis de HbA2 entre 3,4% e 6,8%. Para determinação do genótipo foram utilizadas as técnicas de i) Restriction fragment length polymorphism (RFLP), recorrendo a enzimas de restrição após reação de polimerização em cadeia (PCR); ii) utilização de de sondas TaqMan recorrendo a PCR em tempo real (PCR-real time); iii) sequenciação direta pelo método de Sanger. A análise de regressão linear em modelo aditivo mostrou uma associação estatisticamente significativa com os níveis de HbF para os SNPs BCL11A rs1427407 (p = 0,03), HMIP rs66650371 (p = 0,004) e XmnI-HBG2 rs7482144 (p = 0,004) nos indivíduos portugueses com β-talassemia minor. Os SNPs DCHS2 rs61746132, RNF113B rs16955011, ARHGAP18 rs11759328, KLF1 rs79334031 e LCR-5’HS4 rs16912979 não mostraram associação significativa com os níveis de HbF (p > 0,05). O estudo do efeito cumulativo dos alelos minor dos SNPs BCL11A rs1427407, HMIP rs66650371 e XmnI-HBG2 rs7482144 mostrou que existe um aumento na força de associação aos níveis de HbF com o aumento do número de alelos minor. Em conclusão, os resultados obtidos neste estudo estão de acordo com o que tem sido descrito na literatura para os loci XmnI-HBG2 rs7482144, BCL11A rs1427407 e HMIP rs66650371 associadas à regulação dos níveis de HbF em diferentes amostras populacionais com β-talassemia minor. Todavia, não foi possível identificar novas variantes genéticas envolvidas na regulação de HbF. Considerando que a reativação da expressão do gene HBG2, com o consequente aumento dos níveis de HbF, traria benefícios clínicos importantes para doentes com β-talassemia ou drepanocitose, a identificação de novos loci associados a HbF continua a ser um esforço atual de investigação já que poderá vir a proporcionar a descoberta de novos alvos terapêuticos.In the healthy adult organism, fetal hemoglobin (HbF) represents less than 1% of total hemoglobin, which gives it little clinical importance in normal physiology. However, in certain hemoglobinopathies, increasing the level of HbF may be important as it improves the clinical condition of patients, particularly in patients with sickle cell anemia and β-thalassemia. After understanding how HbF regulation in the adult organism is dependent on genetic mechanisms, paved the way for the investigation and identification of loci involved in the process. To date, several association studies conducted on different population samples have identified three major loci involved in regulating HbF production: the rs7482144 polymorphism, located in the promoter region of the HBG2 gene, the BCL11A gene, and the HMIP intergenic region (HBS1L-MYB). Although, other investigations have identified new polymorphisms that may be involved in varying HbF levels, namely, LCR-5'HS4 rs16912979, KLF1 rs79334031, DCHS2 rs61746132, RNF113B rs16955011 and ARHGAP18 rs11759328. The main objective of this dissertation is to study different candidate loci for HbF level regulators in a sample of Portuguese β-thalassemia minor patients and to test their association with the variation of HbF levels. A total of 66 samples of ortuguese individuals with β-thalassemia minor, 30 male and 36 female, aged 2 to 77 years, diagnosed at the Clinical Hematology Service of the Coimbra Hospital and University Center (CHUC) are studied. The subjects are healthy and asymptomatic, with HbF values between 0.2% and 9% and HbA2 levels between 3.4% and 6.8%. To determine the genotype, the following techniques are used I) Restriction fragment length polymorphism (RFLP) using restriction enzymes after polymerase chain reaction (PCR); ii) use of TaqMan probes using real-time PCR; iii) direct sequencing by the Sanger method. Linear regression analysis in additive model showed a statistically significant association with HbF levels for BCL11A rs1427407 (p = 0,03), HMIP rs66650371 (p = 0,004) and XmnI-HBG2 rs7482144 (p = 0,004) SNPs in Portuguese patients with β-thalassemia minor. SNPs DCHS2 rs61746132, RNF113B rs16955011, ARHGAP18 rs11759328, KLF1 rs79334031 and LCR-5'HS4 rs16912979 showed no significant association with HbF levels (p > 0,05). The study of the cumulative effect of minor alleles of the BCL11A rs1427407, HMIP rs66650371 and XmnI-HBG2 rs7482144 SNPs showed an increase in the strength of association with HbF levels with increasing numbers of minor alleles. In conclusion, the results obtained in this study are consistent with the literature for the XmnI-HBG2 rs7482144, BCL11A rs1427407 and HMIP rs66650371 loci associated with the regulation of HbF levels in different population samples with β-thalassemia minor. However, it was not possible to identify new genetic variants involved in the regulation of HbF.
