Abdominal aortic infectious aneurysm presenting as severe lower limb oedema

Resumo: | A 56 year old man presented with progressive lower limb oedema of 3 months duration. No trauma, infection, pain or other symptoms were refered during this period. Physical examination revealed severe oedema of the whole lower Jimbs and scrotum, and an epigastric and pulsatile, non tender mass, of 8x10 cm. Aortic aneurysm was diagnosed and confirmed by CT scan and aortography. Vena cavagraphy revealed complete obstruction. Surgical correction was performed, during which an exsudate was found in the aneurys1n wall. Staphylococcus aureus was isolated. Dicloxaciline was initiaded. Oedema improved progressively, the patient being asynptomatic 6 months later. ln a thorough literature search on MEDLINE no similar cases were found. |

Os autores descrevem o caso de u1n aneurisma infeccioso (Staphylococcus aureus) da aorta abdominal, com uma forma clínica de apresentação até agora não descrita na bibliografia pesquisada através do MEDLINE: edemas dos membros inferiores e escroto. Após terapêutica cirúrgica e antibioterapia assistiu-se a uma resolução progressiva do quadro clínico, com desaparecimento completo dos edemas.
