Parenting during the child’s hospitalization: model of therapeutic nursing intentions in partnership care

Resumo: | Introduction: Parenting refers the responsibilities of being parents of a child throughout their development. It implies an active and proper role in which the parents feel connected to the child and have the knowledge and skills to fulfill the challenges. Sometimes, the health condition of the child requires hospitalization. Living the experience of having a son hospitalized is often difficult and complex. The parents’ attitude when dealing with their son’s hospitalization depends on the personal characteristics of the son, the parents, and the nurses. In that situation, parents’ attitude also depends on the kind of aid they receive from nursing and the child’s health condition. In pediatric units, setting up a care partnership between nurses and parents is the best pathway for achieving better outcomes. Thus, nowadays, parents and nurses needed care partnership because its benefits are well evidenced. Objectives: What is the principal therapeutical intentionality’s of nurses when promoting the care partnership with parents during the child’s hospitalization? Materials and method: To answer that question, we conducted an action-research project involving nurses from a Portuguese hospital’s pediatric unit. Resulted: Seven types of nurses’ intentions emerged: (1) Promotion of parent’s participation in the care related with the son’s developmental stage (usual care); (2) Promotion of parental competencies to perform complex care; (3) Supervising the parent’s participation in complex care in cases of effective parental’s competencies; (4) Improving on the parents mastery on complex care in cases where was identified a potential for enhancement; (5) Reduction on the level of parental role strain in cases of children with permanent special needs by facilitating, during hospitalization, a period of parents’ rest; (6) Enhancing parents mastery to perform complex care; (7) Enhancing parents mastery in promoting of child’s autonomy. Conclusions: In pediatrics’ nursing practice and in the literature, the concept of the care partnership is achieving more emphasis. We need more research to uncover nurses’ intentions behind care plans involving parents’ partnership. This investigation contributes to clarifying the principal therapeutical intentionality’s of nurses when promoting the care partnership with parents during the child’s hospitalization. |

Isso implica um papel ativo e eficaz em que os pais se sentem ligados à criança e possuem o conhecimento e as habilidades para enfrentar os desafios. Por vezes, a condição de saúde da criança requer um episódio de hospitalização. Viver a experiência de ter um filho internado muitas vezes é difícil e complexo. A atitude dos pais ao lidar com a hospitalização do filho depende das características pessoais do filho, dos pais e dos enfermeiros. Nessa situação, a atitude dos pais depende também do tipo de ajuda que recebem da enfermagem e do estado de saúde da criança. Nos serviços de pediatria, estabelecer parceria de cuidados entre enfermeiros e pais é o melhor caminho para alcançar melhores resultados. Nos dias atuais, pais e enfermeiros necessitam de parceria de cuidados e os seus benefícios são bem evidenciados pela literatura existente. Objetivos: Identificar as intencionalidades terapêuticas dos enfermeiros quando promovem a parceria de cuidados com os pais durante a hospitalização da criança. Materiais e método: Investigação-ação envolvendo enfermeiros do serviço de internamento de pediatria do Hospital Pedro Hispano e pais de crianças hospitalizadas nesse serviço, utilizando-se questionários, análise documental, entrevistas e observação participante. Resultados: Emergiram sete tipos de intenções dos enfermeiros: (1) Promoção da participação dos pais nos cuidados do tipo desenvolvimental (cuidados habituais); (2) Promoção de competências parentais para prestar cuidados complexos; (3) Promoção de participação dos pais nos cuidados complexos quando as competências parentais foram avaliadas como eficazes; (4) Melhoria no desempenho dos pais para realizar os cuidados complexos quando um potencial de valorização de competências dos pais para realizar os cuidados complexos foi identificado; (5) Redução do nível de desgaste associado ao papel parental em pais de crianças com necessidades especiais permanentes, facilitando, durante a hospitalização, o descanso no exercício do papel; (6) Preparação dos pais para prestar cuidados complexos; (7) Preparação dos pais para promover a autonomia da criança. Conclusões: Na prática de enfermagem em pediatria e na literatura, o conceito de parceria de cuidados está amplamente difundido. Contudo, é precisa mais pesquisa para descobrir as intenções dos enfermeiros por trás dos planos de cuidados envolvendo a parceria dos pais.
