However, long waiting times often lead to eligibility loss due to disease progression. Bridging with locorregional therapies has become common practice, but the superiority of one therapy over the others has not yet been established. This research seeks to compare radiological and pathological responses according to the bridging therapy performed, as well as post-transplant survival, recurrence, and quality of life. Materials & Methods: In total, 20 transplanted patients with hepatocellular carcinoma bridged with thermoablation or chemoembolization were included in this study. Patients bridged with more than one type of locorregional therapy were excluded. Radiological response was assessed according to mRECIST criteria and quality of life was evaluated using EORTC QLQ-C30 questionnaire. Results & Discussion: There were no significant differences between radiological and pathological responses between the 2 groups, although the correlation between the mRECIST radiological response and the degree of tumour necrosis in the explant was high, positive and statistically significant in the chemoembolization group. Post-transplant overall survival was, respectively for the group that underwent thermoablation and the group that underwent chemoembolization, 100% vs 81,3% at 6 months; 66,7% vs 81,3% after 1 year; 66,7% vs 75% at 3 years; and 66,7 vs 65,6% at 5 years (p=0,766). Post-transplant recurrence-free survival was 100% in both groups at 6 months; 66,7% in the group undergoing thermoablation vs 100% in the group undergoing chemoembolization after 1 year; and 66,7% for the group bridged with thermoablation vs 90,9% in the chemoembolized group at 3 and 5 years (p=0,127). Post-transplant quality of life between groups varied according to the scale in question, although the difference was not statistically significant. Conclusion: In conclusion, the type of locoregional therapy used as a bridge to transplant apparently had no impact on the degree of imaging and pathological response obtained, post-transplant survival and recurrence, or on overall quality of life.Introdução: A transplantação hepática constitui uma terapêutica potencialmente curativa do carcinoma hepatocelular, particularmente importante naqueles com doença hepática subjacente. Contudo, os longos tempos de espera levam frequentemente a perda de elegibilidade por progressão da doença. A realização de ponte com terapêuticas locorregionais tornou-se prática frequente, mas ainda não foi estabelecida a superioridade de uma das terapêuticas em relação às restantes. Este estudo procura comparar as respostas imagiológica e patológica de acordo com a terapêutica de ponte realizada, bem como a sobrevivência, recorrência e qualidade de vida pós-transplante. Materiais e métodos: No total, 20 doentes com carcinoma hepatocelular transplantados após realização de termoablação ou quimioembolização como ponte foram incluídos neste estudo. Pacientes submetidos a mais do que um tipo de modalidade terapêutica foram excluídos. A resposta imagiológica foi aferida de acordo com os critérios mRECIST e a qualidade de vida avaliada através do questionário EORTC QLQ-C30. Resultados e discussão: Não houve diferenças significativas entre as respostas radiológicas e patológicas entre grupos, embora a correlação entre a resposta imagiológica mRECIST e a necrose tumoral no explante tenha sido elevada, positiva e estatisticamente significativa no grupo submetido a quimioembolização. A sobrevivência global pós-transplante foi, respetivamente para o grupo que realizou termoablação e para o grupo submetido a quimioembolização, de 100% vs 81,3% aos 6 meses; 66,7% vs 81,3% ao fim de 1 ano; 66,7% vs 75% aos 3 anos; e 66,7 vs 65,6% aos 5 anos (p=0,766). A sobrevivência livre de recorrência pós-transplante foi de 100% em ambos os grupos aos 6 meses; 66,7% no grupo submetido a termoablação vs 100% no grupo que realizou quimioembolização ao fim de 1 ano; e 66,7% para o grupo que realizou ponte com termoablação vs 90,9% no grupo quimioembolizado aos 3 e 5 anos (p=0,127). A qualidade de vida pós-transplante entre grupos variou consoante a escala em questão, embora a diferença não tenha sido estatisticamente significativa.
