Observação etnográfica numa unidade de cuidados intensivos neonatais: desafios, ética e trabalho emocional

Resumo: | Over the last decades several legal documents have been produced in order to regulate ethical procedures in scientific research. However, the reflections and possible responses to the challenges emerging from the emotional work performed by qualitative researchers have been neglected, especially when addressing sensitive issues. Based on the reflective analysis of ethically important moments recorded on field diaries during 5 months of ethnographic observation conducted in the Department of Neonatology of a public hospital in the metropolitan area of Porto - Portugal, this article will explore the emotional work of the researcher in managing the challenges provided by the field. The aim is, thus, to contribute to root on fieldwork the limited methodological reflection conducted in this area, particularly in the following levels: self-disclosure and reciprocity regarding the meanings attributed to parenthood; listening to untold stories, management of emotions; and reflexivity arising from the investigator academic background and respective experience of data collection. During the period of ethnographic observation the importance of emotional labor was highlighted. The contact with the parents allowed a deeply understanding of the immensity and magnitude of the ambivalent feelings that they experience in the context of the hospitalization of their children, challenging the normativity of parental roles. The sharing of personal and highly emotional information by mothers and fathers is converted in the responsibility of giving voice to their views, experiences and knowledge in order to contribute to enrich good medical practices, improve health governance and, especially, to the development of parents-centred care in neonatal units. Despite the intersubjectivity underlying ethnographic observation, the establishment of guidelines based on the personal experience of researchers and disseminated in scientific articles, as well as the implementation of training courses oriented to health research involving sensitive contexts and/or issues is advocated. |

Cependant, les réflexions et les possibles réponses aux défis posés par le travail émotionnel réalisé par des chercheurs qualitatifs ont été négligées, en particulier pour approcher thèmes délicats. Basé sur l’analyse réflexive des moments éthiquement pertinents, enregistrés dans les cahiers de terrain plus de 5 mois d’observation ethnographique réalisée au département de néonatalogie d’un hôpital public dans la région métropolitaine de Porto - Portugal, cet article explorera le travail émotionnel du chercheur dans la gestion des défis donnés par le terrain. La prétention avec cet travail ic’est de contribuer ainsi à enraciner dans le travail de terrain la peu réflexion méthodologique faite dans ce domaine, en particulier dans les niveaux suivants: la révélation de soi et de la réciprocité en ce qui concerne les significations attribuées à la parentalité; écouter des histoires verbalisés pour la première fois, la gestion des émotions et la réflexivité due à la formation académique et expérience du chercheur sur la collecte des informations. Au cours de l’observation ethnographique l’importance du travail émotionnel a été soulignée. Le contact avec les parents a permis de mieux comprendre l’immensité et la grandeur de leurs doubles sentiments par rapport à l’hospitalisation de leurs enfants, contre la normativité des rôles parentaux. Le partage d’informations personnelles et franchement émotives par les mères et les pères, devient la responsabilité d’exprimer leurs points de vue, expériences et connaissances afin de contribuer à l’enrichissement de bonnes pratiques cliniques et de gestion de la santé, en particulier pour développer des soins de santé pour les parents. Malgré l’intersubjectivité de l’observation ethnographique, la mise en place de lignes directrices fondées sur l’expérience personnelle des chercheurs et diffusés dans des articles scientifiques est soutenue, ainsi la réalisation des cours de formation sur la recherche en santé sur les contextes et/ou les questions délicates.Ao longo das últimas décadas têm sido produzidos diversos documentos jurídico-legais com o objetivo de regular procedimentos éticos em investigação científica. Contudo, têm sido negligenciadas as reflexões e eventuais respostas aos desafios que decorrem do trabalho emocional desempenhado por investigadores qualitativos, sobretudo quando abordam temas sensíveis. Com base na análise reflexiva de momentos eticamente relevantes registados nos diários de campo ao longo de 5 meses de observação etnográfica realizada no Serviço de Neonatologia de um Hospital público da área metropolitana do Porto – Portugal, neste artigo explorar-se-á o trabalho emocional da investigadora na gestão dos desafios que o terreno lhe foi proporcionando. Pretende-se, desta forma, contribuir para enraizar no trabalho de campo a escassa reflexão metodológica realizada neste domínio, nomeadamente nos seguintes níveis: autorrevelação e reciprocidade no que concerne os significados atribuídos à parentalidade; ouvir histórias verbalizadas pela primeira vez; gestão de emoções; e reflexividade decorrente da formação académica da investigadora e respetiva experiência de recolha de dados. Durante o período de observação etnográfica sobressaiu a importância do trabalho emocional. O contacto com os pais permitiu compreender mais aprofundadamente a imensidade e a magnitude de sentimentos ambivalentes que estes vivenciam no contexto do internamento dos seus filhos, desafiando a normatividade dos papéis parentais. A partilha de informações pessoais e altamente emotivas por parte de mães e pais converte-se na responsabilidade de dar voz às suas visões, experiências e conhecimento, a fim de contribuir para o enriquecimento das boas práticas clínicas e da governação da saúde e, sobretudo, para o desenvolvimento de cuidados de saúde centrados nos pais.
