The COVID-19 pandemic in Brazil: a crisis between science and governance

Resumo: This essay critically addresses the challenges faced in the relationship between Science and healthcare governance in Bra- zil, particularly due to specific issues that have hindered this relationship during the COVID-19 pandemic. During this period, the actions of the federal government’s high-level officials conflicted with healthcare managers from states and municipalities, the scientific community, and the World Health Organization, who relied on scientific evidence to guide actions against the pandemic. The article highlights the implications of this conflict on the governance of the Brazilian pu- blic healthcare system, including normative aspects of decentralization and challenges for the effective implementation of public health policies. Finally, the text presents how collaboration between local healthcare managers, scientific societies, universities, and other civil society organizations was crucial to confront the crisis and overcome the barriers imposed by the federal government.

Au cours de cette période, les décisions des hautes sphères du gouvernement fédéral sont entrées en désaccord avec les responsables de la santé des États et des municipalités, avec la communauté scientifique et avec l'Organisation mondiale de la santé, qui s'appuyaient sur des preuves scientifiques pour guider les actions de lutte contre la pandémie. L'article met en évidence les implications de ce conflit dans la gouvernance du système de santé publique brésilien, y com- pris les aspects normatifs de la décentralisation et les défis pour la mise en œuvre efficace des politiques de santé publique. Enfin, le texte présente comment la collaboration entre les gestionnaires locaux de la santé, les sociétés savantes, les uni- versités et les autres organisations de la société civile a été fondamentale pour faire face à la crise et surmonter les barrières imposées par le gouvernement fédéral.O presente ensaio aborda, de forma crítica, os desafios enfrentados na relação entre a ciência e a governança em Saúde no Brasil, sobretudo em razão de questões específicas que prejudicaram essa relação durante a pandemia de COVID-19. Durante esse pe- ríodo, a atuação do alto escalão do governo federal entrou em conflito com os gestores de saúde de estados e municípios, com a comunidade científica e com a Organização Mundial da Saúde, que se baseavam em evidências científicas para orientar as ações de combate à pandemia. O artigo destaca as implicações desse conflito na governança do sistema público de Saúde brasileiro, incluin- do aspectos normativos de descentralização e os desafios para a implementação efetiva de políticas públicas em Saúde.
