Impact of disease and liver transplantation on quality of life of patients with Cirrhosis: an exploratory study

Resumo: | Hepatic cirrhosis strongly affects quality of life and wellbeing of patients. Hepatic transplantation is the best treatment in the final stages of the disease. This study was designed to evaluate the impact of the disease and hepatic transplantation on the quality of life of cirrhotic patients. A non-probable sample of 61 participants of the Transplant Unit of the Curry Cabral Hospital was elaborated, of which 28 in the active list of transplants,18 transplanted six months ago. Data was obtained through an enquiry interview without using the Liver Disease Quality of Life – LDQOL 1.0 questionnaire, with the respective data. The different dimensions of the quality of life dimensions will be presented: related symptoms illness/transplant; effects of illness/transplant in daily activities; concentration of the quality of the social interaction; wariness with the illness; corporal stigma/image; function and problems to be extracted as the main conclusion. – Hepatic cirrhosis is associated with bad quality of life. This aspect was pointed out in the active preparation for transplant. –- The improvement of the quality of life in the evaluated patients one year after the surgery is less expressive in the group of patients transplanted six months ago. This is more evident in respect of the physical symptoms related to the illness than in the psychosocial aspects – Cirrhosis and hepatic transplant have a strong impact in the quality of life of the patients, the family and the health professionals in their important roles of support and assistance. |

A cirrose hepática é uma doença que afecta gravemente a qualidade de vida e o bem-estar dos doentes; a transplantação hepática constitui o tratamento de eleição na fase final da doença. O objectivo deste estudo foi avaliar qual o impacto da doença e do transplante hepático na qualidade de vida de doentes com cirrose. Foi constituída uma amostra não probabilística de 61 participantes da Unidade de Transplante do Hospital Curry Cabral, dos quais 28 se encontravam em lista activa para transplante, 18 com seis meses de transplante e 15 com um ano de transplante. A colheita de dados foi feita através de inquérito por entrevista semi-estruturada e por aplicação do questionário Liver Disease Quality of Life – LDQOL 1.0., com a respectiva triangulação de dados. Os resultados obtidos nas diferentes dimensões de qualidade de vida: sintomas relacionados com a doença/transplante; efeitos da doença/transplante nas actividades diárias; concentração e memória; qualidade da interacção social; preocupação com o futuro; problemas de sono; isolamento social; preocupação com a doença; estigma/imagem corporal; funcionamento e problemas sexuais permitiram retirar como principais conclusões:- A cirrose hepática está associada a má qualidade de vida, aspecto que se estacou com grande evidência nos doentes em lista activa para transplante. – A melhoria da qualidade de vida nos doentes avaliados após um ano de cirurgia é muito erceptível, sendo menos expressiva no grupo de doentes com 6 meses de transplantação.
