Assessment of the in-hospital course after discharge from an internal medicine service

Resumo: Introduction and methods – Post-discharge follow-up and use of institution resources may be important indicators in the evaluation of a Services’ activity. The authors have retrospectively analyzed some post-discharge assessment parameters in an Internal Medicine Service.Material and Methods – Admissions to the Medicina II Service of the Coimbra University Hospital, who were discharged, as registered on the DRG data-base, , during a 3 month period were selected. Thepatients’ course inside the institution in the following 6 months was studied. Demographic data,outpatient visits, emergency visits and readmissions were analyzed, as were mortality, principaldiagnosis, DRG and its relative weight.Results – 72.9% of all patients had at least one post-discharge contact (outpatient visit, emergency visit, readmission). There were 232 outpatient visits (54.2% of patients), 100 emergency visits (41.6%) and 86 readmissions (41%); of the latter, the most frequent principal diagnosis was pneumonia (10,4%). The mortality rate was 10.4% (15 patients). Some factors influenced the results, like a relative weight over 1.0 and age under 80, which influenced the number of patients with outpatient visits, as well as thediagnosis of pneumonia, which influenced the number of patients with emergency visits and mortality.Conclusions – This study shows a significant impact of the post-discharge care in the same institution, in its diverse sectors. The in-hospital mortality was probably the most important result, revealing thegeneral prognosis of our inpatients. The systematic review of post-discharge assessment parameters,including in-hospital mortality and readmissions is important to overcome the difficulties in today’sInternal Medicine Services. It is necessary, in these studies, to use comparable methods, like DRGs and case-mix.

It is necessary, in these studies, to use comparable methods, like DRGs and case-mix.Introdução e objectivos – O seguimento após a alta hospitalar e respectivo consumo de recursos dentro da mesma instituição poderão ser indicadores importantes na avaliação da actividade dos serviços. Com este objectivo, os autores realizaram um estudo retrospectivo analisando alguns parâmetros de avaliação após o internamento num Serviço de Medicina Interna. Material e métodos – Foram seleccionados os internamentos do Serviço de Medicina II dos Hospitais da Universidade de Coimbra registados na base de dados dos GDH, com alta num período de 3 meses, e estudado o percurso dos doentes dentro da instituição nos 6 meses seguintes à data de alta. Foram analisados dados demográficos, consumo de consultas externas, urgências e novos internamentos e,ainda, a mortalidade, diagnósticos principais, GDH e pesos relativos dos respectivos episódios. Resultados – Destaca-se que 72,9% dos doentes tiveram pelo menos um dos contactos estudados(consulta, urgência, internamento). Registaram-se neste período 232 consultas (54,2% dos doentes), 100 urgências (41,6% dos doentes) e 86 internamentos (41% dos doentes); nestes, o diagnóstico principalmais frequente foi de pneumonia, em 10,4%. A mortalidade foi de 15 doentes no internamento (10,4% dos doentes estudados). Dos factores com influência nos resultados, destacam-se o peso relativo superior a 1,0 dos internamentos iniciais e a idade inferior a 80 anos, reflectindo-se no número de doentes com consulta, assim como o diagnóstico de pneumonia no número de doentes com urgências e mortalidade. Conclusão – Nesta avaliação, constatou-se um impacto significativo de cuidados prestados dentro da mesma instituição, nos seus vários sectores. A mortalidade intra-hospitalar foi provavelmente o resultado mais importante do estudo e revelador do tipo de prognóstico dos doentes internados nos nossosServiços. A revisão sistemática de parâmetros de avaliação após a alta, incluindo o estudo da mortalidade e das readmissões, é importante para contornar as dificuldades hoje existentes nos Serviços de Medicina Interna.
