Giant Epidermoid Splenic Cyst

Resumo: | Introduction: The splenic cysts are rare, with less than 1000 cases reported. Epidermoid cyst accounts for 2.5% of all splenic cysts and 10% of non-parasitic. It is more common in females and usually only cause symptoms when it reaches size> 5cm. It is advocated that these lesions result from the invagination of the splenic capsule’s peritoneal mesothelium with subsequent proliferation and squamous metaplasia. Methods: The authors present a case of a woman aged 27, referred to the emergency room for epigastric pain and bloating the framework of post-prandial, with 2 months of evolution. Upper endoscopic study revealed extrinsic compression of the gastric greater curvature. In the imaging study was documented massive cystic formation in the medial aspect of the spleen, with regular walls and diameter of 14.5 cm, causing extrinsic compression of the gastric greater curvature. Submitted to total splenectomy, Discharged on the 3rd day, with a good postoperative evolution. The anatomopathological examination revealed an epidermoid cyst with 15 cm in greatest diameter. Discussion: The present work aims to draw attention to the rarity of the entity in question, and epidermoid cyst of the spleen is considered in the differential diagnosis of abdominal swelling in young adults. Introdução: Os quistos esplénicos são entidades raras, havendo menos de 1000 casos descritos. O quisto epidermóide corresponde a 2,5% de todos os quistos esplénicos e a 10% dos não parasitários. É mais frequente no sexo feminino, e normalmente só manifesta sintomas quando atinge dimensões>5cm. Preconiza-se que estas lesões resultam da invaginação do mesotélio peritoneal capsular esplénico com subsequente proliferação e metaplasia escamosa. Material e Métodos: Os autores apresentam um caso de uma mulher de 27 anos, referenciada ao serviço de urgência por quadro de epigastralgia e enfartamento pós-prandial, com de 2 meses de evolução. EDA revelou compressão extrínseca da grande curvatura gástrica. No estudo imagiológico foi documentada volumosa formação quística, na vertente medial do baço, de paredes regulares e com diâmetro de 14,5 cm, causando compressão extrínseca da grande curvatura gástrica. Submetida a esplenectomia total, com alta ao 3o dia, apresentando boa evolução pós-operatória. O exame anatomo-patológico revelou quisto epidermóide com 15 cm de maior diâmetro. Discussão: O presente trabalho pretende alertar para a raridade da entidade em questão, devendo o quisto epidermóide do baço ser considerado no diagnóstico diferencial de tumefacção abdominal no adulto jovem. O caso clínico apresentado prima pelas dimensões elevadas e pela sua manifestação tardia. Palavras-chave: quisto esplénico, quisto epidermóide, esplenectomia.
