Social support, anxiety and depression in sarcoma patients

Resumo: Sarcomas are rare malignant tumors, originating from connective tissue or bone tissue. Most frequent treatments for of these tumors are surgical, with the association of chemotherapy and/or radiotherapy. The present study is part of a larger research project, and aims to describe social support and emotional adjustment of patients with sarcoma, as well as their socio-demographic and clinical determinants.A descriptive and transversal study was undertaken in two hospitals, with a sample of 30 patients with sarcomas (soft tissue and bone), with an average of 38 years of age and 10 years of education, mostly married and employed. In clinical terms, a significant part of the sample (n = 16; 53 percent) had already started treatment for the disease, such as surgical exeresis of sarcomas, cutaneous metastases or other surgeries associated with complementary treatments. Instruments used: a demographic and clinical questionnaire, The Social Support Scale and the Hospital Anxiety and Depression Scale. The results indicated that patients had a positive perception about their social support and presented mild values of anxiety. The majority did not show clinical significant values of depression, although some participants evidenced moderate to severe anxiety levels. Some associations were found between social support and emotional adjustment and socio-demographic variables, such as age, educational level, the number of people in the household and the number of children), but not associations were found with clinical variables. This study is constitutes the initial phase of the evaluation of a problem that needs to be deepened, towards the identification of vulnerable groups that require a greater monitoring by health professionals, since the early stages of the disease and its treatment. Nurses must design and implement monitoring processes of these vulnerable groups and implement interventions adjusted to their real needs.

Nurses must design and implement monitoring processes of these vulnerable groups and implement interventions adjusted to their real needs.Os sarcomas são tumores malignos raros, com origem no tecido conjuntivo ou no tecido ósseo, cujo tratamento mais comum é cirúrgico, ao que se associa a quimioterapia e/ou a radioterapia. O presente estudo, inserido no contexto de outro mais alargado, teve como objetivos descrever a perceção de suporte social e de ajustamento emocional dos doentes com sarcomas, bem como os seus determinantes sociodemográficos e clínicos.Desenvolveu-se um estudo descritivo e transversal em dois contextos hospitalares do nosso país, numa amostra de 30 pessoas com sarcomas (ósseos e das partes moles), com uma média de 38 anos de idade e 10 anos de escolaridade, na sua maioria casados e empregados. Em termos clínicos, uma parte significativa da amostra (n=16; 53%) já tinha iniciado tratamento para a doença, como as exéreses cirúrgicas de sarcomas, metástases ou plastias cutâneas, ao que se associavam outros tratamentos complementares. Foram administrados os seguintes instrumentos: um questionário de caracterização sociodemográfica e clínica e as versões portuguesas da Escala de Apoio Social (EAS) e da Escala de Ansiedade e Depressão Hospitalar (EADH).Os resultados indicaram que a amostra tinha uma perceção positiva sobre o seu suporte social e apresentava valores indicativos de ansiedade “ligeira”, não apresentando sintomas de depressão, embora alguns participantes da amostra se encontrassem com níveis de ansiedade moderada a grave, indicativos da necessidade de intervenções urgentes de cariz psicossocial. Foi ainda evidenciada uma relação estatisticamente significativa entre as variáveis em análise e algumas variáveis sociodemográficas (como a idade, a escolaridade, o número de pessoas que coabitavam com os participantes da amostra e o seu número de filhos), mas não com as variáveis clínicas. Consideramos que estes resultados se devam essencialmente ao tamanho diminuto da amostra, pelo que sugerimos a continuidade do presente estudo no sentido de podermos confirmar estas e outras relações entre as variáveis.Julgamos que este estudo se apresenta como um primeiro olhar sobre uma problemática que carece ser aprofundada, no sentido da identificação de grupos vulneráveis e que necessitam de um maior acompanhamento pelos técnicos de saúde, desde as fases iniciais da doença e do seu tratamento.
