It is more frequent in males and with a median age at presentation of 65 years. Classically considered as an aggressive and incurable B-cell lymphoma, it is now known to have a wide spectrum of clinical presentations, ranging from indolent to extremely aggressive.Even in current times, with the increase of information and understanding about this disease, there is no globally accepted guideline for treatment and it is considered an incurable disease. It is generally diagnosed in advanced stages (Ann Harbor stages III and IV), with a mean survival ranging from 20 months (high-risk patients) to a mean survival of 60% in 5 years (low-risk patients).It is a B-cell neoplasm (small to medium-sized centrocyte-like lymphoid cells), originating from B-cells of the peripheral zone of the inner mantle (pre germinal centre).The main feature of MCL is the (11;14) translocation (q13;q22), which results in increased cyclin D1 protein expression and consequent cell cycle dysregulation. MCL presents two main variants: classic MCL which is the most common form and has an overall survival (OS) of approximately 4-5 years; and the blastoid variant which, although rare, has been associated with a more aggressive clinical course and worse prognosis (survival of approximately 15 months). Although some patients have an indolent form, frequently, MCL presents an aggressive clinical course. Although initial therapy may lead to high overall response rates (from 60% to 97%), these are short-lived and most patients relapse.This study aims to investigate which factors have an impact on the response rate to first-line treatment in mantle cell lymphoma.O linfoma de células do manto (LCM) é um subtipo de linfoma não-Hodgkin (NHL) relativamente incomum constituindo cerca de 3-5 a 10% de todos os casos de NHL. É mais frequente em indivíduos do sexo masculino e com idade mediana na apresentação de 65 anos. Sendo classicamente considerado como um linfoma de células B agressivo e incurável, atualmente sabe-se que tem um espetro alargado de apresentações clínicas, variando de indolente até extremamente agressivo.Ainda nos tempos atuais, com o aumento da informação e compreensão sobre esta doença, não há nenhuma guideline de tratamento globalmente aceite sendo considerada uma doença incurável. É geralmente diagnosticado em estádios avançados (III e IV de Ann Harbor), apresentando uma sobrevivência média variando de 20 meses (pacientes de alto risco) até uma sobrevivência média de 60% em 5 anos (pacientes de baixo risco).Trata-se de uma neoplasia de células B (células linfoides pequenas a médias, semelhantes a centrócitos), com origem em células B da zona periférica da zona interna do manto (pré-centro germinativo).A característica principal do LCM é a translocação (11;14) (q13;q22), que resulta no aumento da expressão da proteína ciclina D1 e consequente desregulação do ciclo celular. O LCM apresenta duas variantes principais: o LCM clássico que é a forma mais comum e tem uma sobrevivência global (OS) de aproximadamente 4-5 anos; e a variante blastóide que, embora rara, tem sido associada a um curso clínico mais agressivo e a pior prognóstico (sobrevivência aproximada de 15 meses). Apesar de alguns doentes terem uma forma indolente, frequentemente, o LCM apresenta um curso clínico agressivo.
