Salvage treatment for oropharyngeal squamous cell carcinoma

Resumo: Objectives: Evaluate salvage surgery efficiency for treatment of residual/recurrent oropharyngeal squamous cell carcinoma after radiotherapy or chemorradiotherapy. Methods: Retrospective study that included patients followed at Instituto Português de Oncologia de Lisboa Francisco Gentil, between 2010 and 2021. The study included 160 patients with diagnosis of squamous cell carcinoma of oropharynx, from which 44 patients with  diagnosis of residual/recurrent tumor were selected. Results: Diagnosis of residual/recurrent disease was made in 27,5% of the population initially selected. Between the ones that had local residual/recurrent disease, 8 patients (53,3%) were submitted to surgery, with a three-year overall survival of 50,0%, and the remaining 7 patients did not have favorable surgical conditions. Conclusions: Salvage surgery for treatment of residual/recurrent oropharyngeal squamous cell carcinoma is one of the available treatments with best survival rates. Despite the aforementioned, survival rates are limited and depend on surgical margins, so this kind of treatment should be carefully considered.

Métodos: Estudo retrospetivo de doentes seguidos no Instituto Português de Oncologia de Lisboa Francisco Gentil, entre 2010 e 2021. O estudo incluiu 160 doentes com diagnóstico de carcinoma pavimento-celular da orofaringe, dos quais 44 foram selecionados por se ter objetivado persistência ou recidiva tumoral. Resultados: Ocorreu persistência/recidiva da doença em 27,5% da população inicialmente selecionada. Daqueles que tiveram persistência/recidiva local, 8 doentes (53,3%) foram submetidos a cirurgia, com uma sobrevida global aos 3 anos de 50,0%, sendo que os restantes 7 doentes não tiveram condições cirúrgicas. Conclusão: A cirurgia salvage para tratamento do carcinoma da orofaringe residual/recidivante é um dos tratamentos que apresenta melhores resultados de sobrevida.
