There are three essential factors in communication and physician-patient relationship: empathy, empowerment, and patient satisfaction with information received from the physician about his illness and morbidities. The objective of the present study is to compare the Family physicians and general practitioners and the Hospital physicians in terms of communication and physician-patient relationship in patients with multimorbidity. An exploratory observational cross-sectional study was carried out, using the convenience sample of people with multimorbidity and applying the JSPE, PEI/ICC, MAT and ECsD scales. The statistical treatment of the data was performed using descriptive statistics, the non-parametric test U Mann-Whitney and the Pearson Correlation was used. For all questions on the JSPE scale, the family doctor scored higher than the hospital physician. In the study of the correlation between the specialties, statistically significant differences were obtained. In the PEI/ICC scale of the Family physicians and general practitioners, the "better" response was predominant. In the MAT scale, an average of 39.1 was obtained. The predominant response in the ECsD questionnaire was "I know what is needed". The Pearson correlation coefficient between the JSPE and PEI/ICC scales is positive and moderate, between the PEI/ICC and MAT scales is positive and weak and between the JSPE and MAT scales is negative and almost 0. The differences between the Family physicians and general practitioners and the Hospital consultations have an impact on the results of the medical empathy felt by the patients. The results of the MAT scale demonstrate an excellent adherence to drug therapy. In the correlation between empathy, empowerment and adherence to treatment, it was verified that a higher degree of empathy is correlated with a higher degree of empowerment and that a higher degree of empowerment is correlated with a higher degree of adherence to treatment.Uma comunicação eficaz na relação médico-doente está diretamente relacionada com a promoção da saúde dos pacientes com multimorbilidades que representam 70% dos pacientes que frequentam os Cuidados de Saúde Primários. Há três fatores imprescindíveis na comunicação e relação médico-doente: a empatia, a capacitação e a satisfação do doente perante a informação recebida do médico sobre a sua doença e morbilidades. O objetivo do presente estudo é comparar as consultas de Medicina Geral e Familiar e as consultas de especialidades Hospitalares em termos de comunicação e relação médico-doente em pacientes com multimorbilidade. Foi realizado um estudo transversal observacional exploratório, recorrendo a amostra de conveniência de pessoas com multimorbilidade e aplicando as escalas JSPE, PEI/ICC, MAT e ECsD. Foi efetuado estudo estatístico descritivo, teste não-paramétrico U de Mann-Whitney e Correlação de Pearson. Para todas as questões da escala JSPE o médico de família obteve pontuações mais elevadas do que o médico hospitalar. No estudo da correlação entre as especialidades obteve-se diferenças estatisticamente significativas. Na escala PEI/ICC das consultas de Medicina Geral e Familiar obteve-se predomínio da resposta “melhor”. Na escala MAT obteve-se uma média de 39,1. A resposta com predomínio no questionário ECsD foi “sei o necessário”. O coeficiente de correlação de Pearson entre as escalas de JSPE e PEI/ICC é positivo e moderado, entre as escalas de PEI/ICC e MAT é positivo e fraco e entre as escalas JSPE e MAT é negativo e quase 0.As diferenças entre as consultas de Medicina Geral e Familiar e as consultas Hospitalares têm impacto nos resultados da empatia médica sentida por parte dos pacientes. Os resultados da escala MAT demonstram uma ótima adesão à terapêutica medicamentosa por parte dos inquiridos.
