Vantagens da presença dos doutores palhaços no contexto hospitalar: as expectativas dos profissionais de pediatria

Resumo: Focused on interventions that - affiliated to the hospitals’ humanization process - search for the augmentation of children’s quality of life inside the hospital, and the minimization of the negative effects of this experience on their welfare and development, special attention is given to interventions that combine art, play, and humor. Music, storytelling, magic and clowns are examples of these interventions. Amongst them, the authors emphasize the work of “Operação Nariz Vermelho”, a 10 years Portuguese association of professional clowns. In 2010 “Operação Nariz Vermelho” invested on a 12th partner: Hospital de Braga. Simultaneously, an additional challenge was initiated: the research project “Rir é o melhor remédio?” (resulting from a research protocol with the Minho University’s Institute of Education), aimed at the study of the Clown Doctors’ interventions. To begin with, a two-stage study was designed. The first stage (one month before starting intervention in this pediatric unit) -, focused on the evaluation of staff’s expectations regarding clowns’ intervention and, at a second stage (still running), their representations around the clowns’ work two years after observing and cooperating with them. Main results of the first stage (34 professionals’ interviews) are presented, as well as the second stage’s preliminary results. Data show that, in a first stage, the staff shows considerable interest and openness regarding the presence of clowns in their unit, and regarding the possibility of working together. Several advantages for the children, parents, pediatric staff, and to the hospital itself were foreseen, for example: the clowns’ moderating impact on the children’s’ internment experience; more cooperation with routines and procedures, or the facilitation of family-staff communication. The children’s fear of clowns; the interpretation of CDs’ presence as “childish” (for teens), and/or a “lack of respect” for the disease and the children’s (and families) suffering were anticipated as main difficulties.

Avec l’augmentation graduelle des taux de survie, on ne fait plus, désormais, attention exclusivement à la mort éminente de l’enfant mais on considère le cancer pédiatrique comme une maladie chronique prolongée. Les expériences associées à la maladie oncologique (ex. les hospitalisations fréquentes; les restrictions au niveau de l’autonomie; l’éloignement de la famille et des amis ; la soumission aux procédures et aux traitements douloureux et invasifs) bouleversent le bien-être et le développement infantile, et obligent que l’enfant/adolescent et sa famille fassent de nombreuses adaptations. La façon dont sont faites cette confrontation et cette adaptation chez l’enfant et sa famille est un important thème d’étude. Ce travail, ayant un caractère exploratoire, analyse la dimension phénoménologique et les répercussions psychosociales de la maladie oncologique pédiatrique chez les enfants/adolescents et leurs parents, mais plus particulièrement les difficultés, les besoins et préoccupations vécus depuis le diagnostique jusqu’à la phase terminale. En même temps, on essaye d’analyser les moyens d’interventions existants dans les services d’oncologie pédiatrique que l’on étudie et son apport pour la promotion du bien-être. On a fait 30 interviews semi structurées avec des professionnels de santé, de l’éducation, du service social et de la psychologie qui accompagnent directement ces familles dans les services ambulatoires et d’internements de l’hôpital de S. João et de l’Institut Portugais d’Oncologie du Porto. On a réalisé, également, trois groupes de discussions avec des clowns d’hôpital professionnels de l’ Operação Nariz Vermelho. Ceux-ci ont aussi permis d’analyser leurs perceptions vis-à-vis des préoccupations, des besoins et des difficultés de ces familles. L’option que nous avons faite d’un abordage indirect à la phénoménologie de ces procès se doit à des principes éthiques : le besoin de respecter l’intégrité et la souffrance des personnes impliquées. Une fois que nous avons transcrit toutes les interviews, nous avons commencé à traiter et à analyser les données, ayant comme support le programme informatique d’analyse qualitative WEBQDA. Nous présentons, ici, les résultats préliminaires de cette étude.Entre as abordagens que, no âmbito do movimento de humanização hospitalar, procuram ampliar a qualidade do internamento pediátrico, ou, minimizar o seu impacto negativo no bem-estar e desenvolvimento da criança, destacamos aquelas que aliam a arte, o lúdico, e o humor. Delas são exemplo a música, o conto, a magia, ou os palhaços de hospital. Neste trabalho é dada ênfase à intervenção dos palhaços de hospital, em particular à que tem vindo a ser desenvolvida, em Portugal, nos últimos 10 anos, pela Operação Nariz vermelho. Em 2010, esta associação de palhaços profissionais iniciou a sua colaboração com um 12º parceiro: o Hospital de Braga. Numa tentativa de conhecer as expectativas e representações dos profissionais dos serviços de pediatria deste hospital em torno do trabalho dos Doutores Palhaços, o projeto de investigação “Rir é o melhor remédio?” (uma parceria com o Instituto de Educação da Universidade do Minho) desenvolveu um estudo bi-etápico focado no olhar destes profissionais. Inicialmente foram exploradas as suas expectativas relativamente à intervenção dos palhaços; explorando-se, numa segunda etapa (ainda em curso), as representações construídas ao longo de dois anos de trabalho em comum. Nesta comunicação apresentam-se os principais resultados da primeira etapa (entrevistas com 34 profissionais), e os resultados preliminares da segunda. O primeiro estudo revela a grande abertura da generalidade dos profissionais em relação à parceria com os palhaços, antecipando várias vantagens para a criança, seus pais, profissionais dos serviços de pediatria, e, para o próprio hospital. A amenização da experiência de internamento, uma maior cooperação com as rotinas e tratamentos, ou, a facilitação da comunicação profissionais-família foram referidas como potenciais vantagens.
