Inquérito sobre a bilharziose numa área limitada do distrito do NiassaParasitologia médicaBilharzioseEstudo de casoNiassaMoçambiqueContém quadros e mapas.Le présent raport a pour but de contribuer à la connaissance de l’état actuel de la bilharse dans une région limitée du district du Nyassa dans laquelle est instalée la Section de Santé de la Brigade de Construction du Chemin de Fer du Mozambique. La région étudiée posséde des conditions climatiques convenables à la prévalence de cette endémie, si bien que, orogéologiquement, l’ambiance ne soit tellement favorable. Quoique asymptomatique dans la plupart des cas, la bilharziose atteint un haut pourcentage d’individus, surtout sous sa forme vésicale. Par les éléments antérieurs on apprend que la variété vésicale affecte 40,4% des individus et l’intestinale 15,6%. Le sexe féminin est le responsable par 40% des cas et le masculin par les 60%, dans la forme vésicale. En ce qui concerne la variété intestinale, la distribution est sensiblement égale, 39% et 61% respectivement. Les cas les plus graves sont tombés sur le groupe 6 – 10 ans. Les individus les plus jeunes chez lesquels on a diagnostiqué la maladie étaient ages de 1,5 ans (Sch. mansoni) et 3 ans (Sch. haematobium). Nous n’avons pas trouvé le Sch. haematobium dans les fèces ni le Sch. Mansoni dans l’urine. Les indices cliniques et d’infection humaine, quoique élevés, sont inférieurs à ceux d’autres zones endémiques du continente africain. Cela est du à la réduite densité démographyque, à la qualité des cours d’eau et à l’absence d’aires marécageuses, de fossés de drénage, etc. Dans l’aggloméré de Belém, lequel, en dépit de son insignifiance, est le troisème de ce district, les taux d’infection son sensiblement inférieurs à ceux des localités limítrofes peulées par des gens de mentalité très inférieurs. Cela nous prouve la valeur de l’education sanitaire pour l’éradication de cette endémie, éducation difficile même chez les peuplements plus évolués. Ainsi, à Belém, l’incidence de la forme vésicale est de 38,8% et celle de la forme intestinale de 10,7% pour une étude de 170 et 150 individus, respectivement. Étant donnée l’identité de conditions naturelles, il se peut que les índices déterminés dans la région de Belém soient similaires dans tout le district du Nyassa. En ce qui concerne les mollusques vedeurs, leur densité est petite et lavariété est réduit à quatre espèces.The present work is intended as a contribution for the knowledge of current status of bilharziasis in a limited area of the Nyassa district where the Health Section of the Mozambique railroad Brigade is installed. The region observed presents climatic conditions which are favourable to the prevalence of the endemia, though the orogeological environment is unfavourable. Though asymptomatic in most cases, bilharziosis appears in a large percentage of individuals, mainly in the vesical form. According to previously obtained data, it was seen that the vesical form affects 40,4% of the individuals, and the intestinal form affects 15,6%. The female sex is responsible for 40% of the cases and the male for the remaining 60%, in the vesical form. As for as the intestinal form is concerned, the distribution is rather similar, with 39% and 61% respectively. The most serious cases were reported in the 6-10 years age group. The youngest individuals in whom the disease was diagnosed were 1,5 years old (Sch. mansoni) and 3 years old (Sch. haematobium). No Sch. haematobium was found in the feces, nor Sch. mansoni in the urine. The clinical and human infection indices, though high, are lower than the ones of other endemic zones of the African Continent. This facts is due to the reduced demographic density, to the quality of the water streams and to the absence of swampy areas, drainage ditches, etc.. In the population cluster of Belém, which, though insignificant, is the third of the district, the infection rates are sensibly lower than the ones of the neighbouring villages, which are inhabited by peoples with a much lower mentality. This fact evidences the importance, concerning the eradication of the endemia, of the sanitary education, which is rather difficult even in the more evolved populations. Thus, in Belém, the incidence of the vesical form is 38,8% and the intestinal form is 10,7%, in the observation of 170 and 150 individuals respectively. Due to the identify of the natural conditions, the indices obtained in the Belém region are apt to be similar in the whole Nyassa district.
