Internacionalmente, enquanto vários países tentam promover a aprovação de novos quadros legislativos mais inclusivos para pessoas LGBTI+, vários relatórios continuam a sublinhar a persistência de desigualdades sociais dentro da comunidade, incluindo desigualdade em saúde.Embora este desafio seja uma luta comum dentro da comunidade LGBTI+, o acrónimo engloba dentro de si uma diversidade de identidades definidas em torno de conceitos de género e sexualidade. Consequentemente, daqui emergem experiências e relações de poder diferenciadas que implicam graus distintos de desigualdade em saúde. Dado isto, as pessoas transgénero são mais propensas a apresentar piores resultados em termos de saúde, sofrendo violações mais frequentes dos seus direitos (cerca de 54% versus 47% em comparação com os seus homólogos LGB), apresentando maiores dificuldades em aceder a cuidados de saúde adequados, maior exposição a fatores de stress social e risco de invisibilidade dentro das agendas de políticas públicas e investigação dado o seu estatuto de “minoria dentro de uma minoria”. Entretanto, com as crescentes mudanças nas paisagens culturais, económicas, políticas e sociais de cada país, novas questões emergem sobre a forma como as desigualdades sociais em comunidades vulneráveis, como a comunidade transgénero, devem ser abordadas, principalmente quando os mecanismos de governação tradicional não são suficientes para responder ao aumento da desigualdade. Atendendo ao contexto apresentado, a seguinte dissertação investiga a forma como os movimentos sociais, principalmente os movimentos sociais transgénero, abordam a desigualdade em saúde dentro da sua comunidade. Partindo de uma análise interdisciplinar entre estudos sobre movimentos sociais, desigualdade social e estudos queer / transgénero, argumentamos que o ativismo social inscrito nos movimentos sociais transgénero é uma ferramenta que permite criar, redefinir e atribuir significados às experiências coletivas e individuais de saúde e sua desigualdade pela comunidade. Consequentemente, expomos que os movimentos sociais constroem significados alternativos em torno da saúde transgénero e sua desigualdade revelando a sua natureza polissémica e alterando os significados hegemónicos atribuídos aos mesmos. Recorrendo à triangulação de métodos (entrevistas semiestruturadas, análise documental e análise temática reflexiva) para analisar uma amostra de associações transgénero em Portugal e França, os nossos resultados revelaram quatro temas centrais (cuidados de afirmação do género, cuidados de saúde mental, cuidados sexuais e reprodutivos e saúde social). Na sua generalidade, os nossos resultados demonstram que a saúde transgénero e a sua desigualdade expandem-se para além das suas compreensões hegemónicas, enfatizando uma rede complexa de determinantes sociais de saúde responsáveis pela saúde desta população que delineia diversos desafios, preocupações e necessidades de saúde da mesma. Para além de a nossa investigação demonstrar o papel transformador dos movimentos sociais transgénero em questões de desigualdade em saúde, ela demonstra como os movimentos sociais podem alavancar a produção de investigação sobre as desigualdades em saúde transgénero, salientando lacunas existentes dentro da mesma. Por fim, eles reinserem a desigualdade em saúde dentro do corpo político e nas estruturas mais amplas da sociedade, indicando a necessidade de repensar a delineação de políticas públicas para a saúde.Recently, LGBTI + rights, primarily transgender rights, have remarkably advanced. Internationally, while countries try to promote an inclusive legislative framework for LGBTI+ people, reports continue to demonstrate the community continues to experience social inequality, including health inequalities. Although health inequality is a common struggle in the community, the acronym LGBTI+ comprises various identities defined around gender and sexuality. Such can translate into differentiated power relations, experiences, and, consequently, distinct degrees of health inequality. Concerning this, transgender populations are more likely to face worse health outcomes, experiencing more frequent violations of their rights (around 54% versus 47% compared to their LGB counterparts), difficulties in accessing appropriate health care, exposure to social stressors and risk of invisibility in research and policy.Meanwhile, with increasing changes in cultural, economic, political, and social landscapes, questions arise about how health inequalities in vulnerable communities such as the transgender community can be addressed, primarily when traditional governance does not suffice to respond to increasing inequalities. Drawing inspiration from social movements and critical approaches, our research investigates how social movements, primarily transgender social movements, are valuable mechanisms to address health inequalities in their community. By drawing from interdisciplinary scholarship on social movements, social inequality, and queer/transgender studies, we advance that social activism inscribed in transgender social movements serves as a tool for creating, redefining, and assigning meanings attached to their collective and individual experiences of health and its inequality. Consequently, we argue that social movements build alternative meanings around transgender health and inequality by unveiling its polysemic nature and shifting hegemonic understandings about these concepts. To achieve this, we deploy method triangulation (use of semi-structured interviews, document analysis and reflexive thematic analysis) and apply it to the study of a sample composed of Portuguese and French transgender associations. Our results revealed four central themes built around transgender health (gender-affirming care, mental health care, sexual and reproductive care, and social health), which unveil the polysemic and shifting meanings of transgender health.
