Intervenção “Proximidade Geriátrica”: um contributo para o envelhecimento ativo e saudável na pessoa idosa institucionalizada

Resumo: | Breve Fundamentação: As alterações demográficas, epidemiológicas e sociofamiliares que caracterizam a mudança na População Portuguesa e Mundial devem ser vistas como inevitáveis e alvo de intervenção. Constatamos a existência de um número crescente de pessoas com mais de 65 anos, com multimorbilidade associada e níveis de dependência elevados e que necessitam de cuidados. De facto, a esperança média de vida aumentou, mas não foi acompanhada por um aumento da esperança média de vida com saúde. Estes fatores, aliados às alterações sociofamiliares, como por exemplo, a passagem das famílias alargadas para as famílias nucleares, veio alterar o processo assistencial, pelo que se tem recorrido cada vez mais à institucionalização. A institucionalização, por sua vez, leva muitas das vezes à quebra das relações sociais e sociofamiliares dos idosos. Fenómeno que vem sendo agravado pela situação pandémica, resultante por exemplo da suspensão de visitas nos Lares. Torna-se assim necessário e urgente reativar as relações sociais sociofamiliares, minimizando o impacto na saúde, pelo que se pretende implementar uma intervenção de Proximidade Geriátrica, de forma a contribuir para um Envelhecimento Ativo e Saudável. Objetivo: O objetivo deste estudo de investigação foi avaliar o impacto da implementação de uma intervenção de Proximidade Geriátrica na funcionalidade, na satisfação com o suporte social, na experiência afetiva e no sentimento de solidão das pessoas idosas institucionalizadas. Metodologia: Foi realizado um estudo quasi-experimental de natureza quantitativa, com uma amostra de 34 pessoas com mais de 65 anos residentes em Estruturas Residenciais para Idosos (ERPI). O grupo experimental (GE) foi constituído por idosos da instituição A e o grupo de controlo (GC) foi constituído por idosos da instituição B que aceitaram participar. O GE foi sujeito à intervenção “Proximidade Geriátrica” e o GC manteve, os cuidados como habitualmente, sem ser sujeito à intervenção. A intervenção consistiu em 10 sessões grupais que se centraram no reavivar de memórias passadas, com temas distintos. Foram aplicados quatro instrumentos de avaliação a ambos os grupos, a Escala da Satisfação com o Suporte Social (ESSS); Elderly Nursing Core Set (ENCS); Escala de Afeto Positivo Afeto Negativo - versão reduzida (PANAS-VRP) e Escala da Solidão UCLA. Foram feitas avaliações no início, durante e no fim do estudo. A fase de recolha de dados e intervenção teve a duração de 8 semanas. Foram realizadas análises estatísticas descritivas e inferenciais com recurso ao SPSS, tendo em conta os pressupostos de normalidade dos dados. Resultados: Ambos os grupos foram constituídos por 17 indivíduos cada sendo a maioria no GE do sexo masculino (64.7%) e no, GC do sexo feminino (64.7%). 61.8% da amostra global, tem o estado civil de viúvo/a. No global, as mulheres são mais velhas que os homens. Em relação aos scores globais das diferentes escalas o GC não apresenta diferenças entre os 3 instantes de medição, enquanto o GE, apresenta entre a primeira e a terceira medições (todos os p<0.05), observando-se uma redução da pontuação do perfil funcional geral, da escala de solidão e do afeto negativo e um aumento da satisfação com o suporte social e do afeto positivo. Conclusão: A intervenção “Proximidade Geriátrica” traz benefícios para as pessoas idosas institucionalizadas melhorando a satisfação com o suporte social, a funcionalidade ao nível da cognição e a afetividade, diminuindo os sentimentos de solidão. Conclusion: The intervention “Geriatric Proximity” brings benefits to institutionalized elderly people, improving satisfaction with social support, functionality at the level of cognition and affectivity, reducing feelings of loneliness. However, we understand that for an effective improvement in communication and self-care, a set of procedural and structural changes in institutions is necessary, with the application of a model of continuous and integrated care. |
