Promoting a healthy life style and the self-management of the disease in employees with type 2 Diabetes

Resumo: Diabetes Mellitus is a chronic disease that is becoming more frequent in Portugal and worldwide. It is characterized by an increase in blood glucose levels – hyperglycemia. In the case of Diabetes Mellitus Type 2, such increase is due to insufficient production of insulin or to inefficient use of the insulin produced by the organism. The current community intervention project is being developed at the Occupational Health Service. Our goal is to encourage behavioral change and promote healthy habits of self-management of the disease. This work is grounded on the Transtheoretical Model of Change, and followed the methodology of the Health Planning Process. The following diagnostic tools were used: the Diabetes Self-Care Activities Scale and the Diabetes Knowledge Questionnaire (DKQ-24) both translated by Bastos et al. These measures were administered to 20 employees with Diabetes Mellitus Type 2, ten of which withdrew, leaving the final sample with 10 participants. Using the Hanlon Method we prioritized the issues identified. These issues were later categorized according to the International Classification for Nursing Practice ® Version 2. Given that this intervention was developed in an Occupational Health setting, we decided to use a methodology directed to groups, following the health education strategy, addressing three learning domains: cognitive, affective and psychomotor. The evaluation of the intervention according to process, compliance and result indicators is positive.

É caracterizada pelo aumento dos níveis de açúcar no sangue – hiperglicemia –, sendo no caso da Diabetes Mellitus tipo 2 devido a produção insuficiente de insulina ou utilização ineficaz da insulina produzida pelo organismo. A finalidade da intervenção comunitária, realizado em setting Serviço de Saúde Ocupacional, visa promover hábitos saudáveis nos profissionais de saúde portadores de Diabetes Mellitus tipo 2, pressupondo alterações de comportamento que permitam desenvolver a autonomia e maior eficácia na gestão da doença. Todo o trabalho desenvolvido foi consubstanciado na metodologia do Processo de Planeamento em Saúde e no Modelo Transteórico da Mudança. Para o diagnóstico de situação utilizámos como instrumentos: Escala de Autocuidado com a Diabetes e Questionário de Avaliação de Conhecimentos (DKQ – 24). Fazem parte da amostra, 10 trabalhadores portadores de Diabetes Mellitus tipo 2. Pelo método de Hanlon, priorizámos os problemas identificados que traduzimos em diagnósticos de enfermagem enunciados na taxonomia CIPE® Versão 2. Optámos pela estratégia de Educação para a Saúde, considerando os três domínios da aprendizagem.
