Clinical evaluation protocol for chronic rhinosinusitis with polyposis during treatment with biological agents

Resumo: Introduction: Chronic rhinosinusitis with nasal polyposis is a common pathology in Otorhinolaryngology, and the therapeutic options, in a simplified way, are medical and/or surgical. Medical therapeutic options, in addition to topical and oral corticosteroid therapy, include the use of biological agents. Three biological agents were recently approved for the treatment of chronic rhinosinusitis with nasal polyposis in Portugal, being indicated in critically ill patients not controlled with topical nasal corticosteroid therapy, for whom surgery (unless contraindicated) and/or systemic corticosteroid therapy did not provide adequate control of the disease. Objectives: To propose a clinical evaluation protocol in patients under biological treatment. Methods: Review of relevant medical literature, namely the two main international working groups on this topic: European Position Paper on Rhinosinusitis and Nasal Polyps 2020 (EPOS) and European Forum for Research and Education in Allergy and Airway Diseases (EUFOREA). Results: The protocol created can be divided into 3 phases: an initial phase with collection of demographic and clinical data, a second phase with evaluation of eligibility for biologicals based on well-defined admission criteria and a third phase with a proposal of follow-up and application of efficacy and discontinuation criteria. Conclusion: This clinical protocol presents a proposition for the collection of standardized data to be used in clinical practice, which allows a uniform collection for carrying out prospective and/or retrospective multicenter studies, in addition to a proposition for follow-up and evaluation of the efficacy/failure of the treatment with biological agents, in patients with chronic rhinosinusitis with polyposis.

As opções terapêuticas médicas, além da corticoterapia tópica e oral, englobam a utilização de agentes biológicos. Foi recentemente aprovada a Autorização de Introdução no Mercado Português de três agentes biológicos para o tratamento da rinossinusite crónica com polipose nasal, estando indicados em doentes graves e não controlados com corticoterapia tópica nasal, para quem a cirurgia (excepto se contraindicada) e/ou corticoterapia sistémica não forneceu controlo adequado da doença. Objetivos: Propor um protocolo avaliação clínica em doentes que se encontram sob tratamento com biológicos. Métodos: Revisão da literatura médica pertinente, nomeadamente dos dois principais grupos   de trabalho internacionais sobre este tema: European Position Paper on Rhinosinusitis and Nasal Polyps 2020 (EPOS) e European Forum for Research and Education in Allergy and Airway Diseases (EUFOREA).   Resultados: O protocolo criado pode ser divido em 3 fases: uma fase inicial com colheita de dados demográficos e clínicos, uma segunda fase com avaliação de elegibilidade para biológicos com base em critérios de admissão bem definidos e uma terceira fase com uma proposta de seguimento e aplicação de critérios de eficácia e de suspensão da terapêutica.
