Muscle strengthening and increasing the articular amplitude of the person submitted to total knee arthroplasty: Case study

Resumo: Introduction: Goonarthrosis is characterized by the wear of the articular cartilage of the knee and is manifested by disabling pain, joint stiffness and muscle atrophy. Total knee arthroplasty is a possible treatment, which aims to alleviate pain, improve joint range and functionality. However, it requires a rehabilitation plan that includes a set of therapeutic interventions in order to provide muscle strengthening and increase the range of motion of the knee, ensuring independence in life activities and improving the person's functionality. Objective: To identify the impact of rehabilitation nursing care in terms of muscle strength and joint range of motion in a person undergoing total knee arthroplasty. Methodology: Case study that complies with the CAse REport guidelines, in which the case of a person undergoing total knee arthroplasty is presented. The Medical Research Council modified muscle strength scale, goniometry, Barthel Index and Timed up and go test were used as assessment instruments. Results: There were gains in increasing the range of motion of the knee from 55º to 85º, in muscle strength of the hip and left knee joint segments, and in the person's functional independence, obtaining a total score of 95 in the Barthel Index and a time of 38 seconds in the Timed up and go test. Conclusion: The implemented rehabilitation plan contributed beneficially to the recovery of the person undergoing total knee arthroplasty.

La artroplastia total de rodilla es un posible tratamiento, cuyo objetivo es aliviar el dolor, mejorar el rango y la funcionalidad de las articulaciones. Sin embargo, requiere un plan de rehabilitación que incluya un conjunto de intervenciones terapéuticas con el fin de proporcionar fortalecimiento muscular y aumentar la amplitud de movimiento de la rodilla, asegurando la independencia en las actividades de la vida y mejorando la funcionalidad de la persona. Objetivo: Identificar el impacto del plan de rehabilitación en términos de fuerza muscular y rango articular, en una persona que se somete a una artroplastia total de rodilla. Metodología: Estudio de caso que cumple con la guía CAse REport, en el que se presenta el caso de una persona sometida a una artroplastia total de rodilla. Se utilizaron como instrumentos de evaluación la escala de fuerza muscular modificada del Medical Research Council, la goniometría, el índice de Barthel y la prueba Timed up and go. Resultados: Se obtuvieron ganancias en el aumento del rango de movimiento de la rodilla de 55º a 85º, en la fuerza muscular de los segmentos de la articulación de la cadera y rodilla izquierda, y en la independencia funcional de la persona, obteniendo una puntuación total de 95 en el Índice de Barthel y un tiempo de 38 segundos en la prueba Timed up and go test. Conclusión: El plan de rehabilitación implementado contribuyó de manera beneficiosa a la recuperación de la persona sometida a artroplastia total de rodilla.Introdução: A gonartrose caracteriza-se pelo desgaste da cartilagem articular e manifesta-se por dor incapacitante, rigidez articular e atrofia muscular. A artroplastia total do joelho é uma opção terapêutica, que tem a finalidade de aliviar quadro álgico, melhorar a amplitude articular e a funcionalidade. Requer, contudo, um plano de reabilitação que inclua um conjunto de intervenções terapêuticas de modo a proporcionar o fortalecimento muscular e o aumento da amplitude articular do joelho, que garantam a independência nas atividades de vida e a melhoria da funcionalidade da pessoa. Objetivo: Identificar o impacto do plano de reabilitação ao nível da força muscular e da amplitude articular, numa pessoa submetida a artroplastia total do joelho. Metodologia: Estudo de caso que respeita as guidelines CAse REport, em que se apresenta o caso de uma pessoa submetida a artroplastia total do joelho. Foram utilizados como instrumentos de avaliação, a escala de força muscular modificada da Medical Research Council, goniometria, Índice de Barthel e Timed up and go test. Resultados: Verificou-se ganhos no aumento da flexão do joelho de 55º para 85º, na força muscular dos segmentos articulares coxofemoral e joelho esquerdo e na independência funcional da pessoa, obtendo um score total de 95 no Índice de Barthel e um tempo de 38 segundos no Timed up and go test.
