Modern orthopedics: the use of robotics in total knee arthroplasty

Resumo: | Introduction: The advance in the development of implant technologies that are more resistant and more modern, adapting the human anatomy, reducing pain and the readaptation process, in addition to the great improvement of surgical techniques and techniques of sterilization and packaging of surgical materials, made arthroplasty knee (TKA) the most performed group of surgeries in the orthopedic environment. The advancement of techniques in the application of knee trampling has made the technique of frequent and increasing use in the last decade, conditioning its success to several factors: the design of the components, the quality of the available materials, the manufacturing technique, application adequate to each patient, as well as the development of the surgical technique. Methodology: This is a narrative review of the literature. Results: A great instrument is the navigation of total knee arthroplasty, which is an instrument that facilitates the operative performance, also having the ability to assist the surgeon in making intraoperative decisions, enabling the simulation of situations before performing them. It has great use in the teaching field, as it makes it possible to immediately see the effect of each action performed during the operation, and it is possible that the navigator eliminates the need for the surgeon's qualification or reduces the value of his experience, as the navigator does not point out paths or make decisions. by the surgeon, providing only assistance in obtaining and measuring the planned results for each action. This activity enhances the surgeon's knowledge and experiences as it provides great parameters with high precision and real-time objectives in the operation. Conclusion: The use of robotics in orthopedic surgery should be seen as an instrument available to the Surgeon, demonstrating their knowledge and never as an autonomous independent system that can compensate for an eventual deficiency of experience or knowledge. Important studies reveal that using robotics, surgery revision rates have decreased, increasing patient satisfaction with the surgery. |

El avance de las técnicas en la aplicación del pisoteo de rodillas ha hecho que la técnica sea de uso frecuente y creciente en la última década, condicionando su éxito a varios factores: el diseño de los componentes, la calidad de los materiales disponibles, la técnica de fabricación, la aplicación adecuada a cada paciente, así como el desarrollo de la técnica quirúrgica. Metodología: Se trata de una revisión narrativa de la literatura. Resultados: Un gran instrumento es la navegación de artroplastia total de rodilla, que es un instrumento que facilita la actuación operatoria, teniendo además la capacidad de asistir al cirujano en la toma de decisiones intraoperatorias, posibilitando la simulación de situaciones antes de realizarlas. Tiene una gran utilidad en el campo de la docencia, ya que permite ver inmediatamente el efecto de cada acción realizada durante la operación, y es posible que el navegador elimine la necesidad de la calificación del cirujano o reduzca el valor de su experiencia, ya que el navegante no señala caminos ni toma decisiones por parte del cirujano, brindando únicamente asistencia en la obtención y medición de los resultados planificados para cada acción. Esta actividad potencia los conocimientos y experiencias del cirujano ya que proporciona grandes parámetros con alta precisión y objetivos en tiempo real en la operación. Conclusión: El uso de la robótica en cirugía ortopédica debe ser visto como un instrumento a disposición del Cirujano, demostrando su conocimiento y nunca como un sistema autónomo e independiente que pueda suplir una eventual deficiencia de experiencia o conocimiento. Importantes estudios revelan que con el uso de la robótica, las tasas de revisión de la cirugía han disminuido, aumentando la satisfacción del paciente con la cirugía.Introdução: O avanço no desenvolvimento de tecnologias de implantes que sejam mais resistentes e mais modernas adaptando a anatomia humana diminuindo a dor e o processo de readaptação, além do grande aperfeiçoamento de técnicas cirúrgicas e técnicas de esterilização e acondicionamento dos materiais cirúrgicos, tornou a artroplastia de joelho (ATJ) o grupo de cirurgias mais realizadas no meio ortopédico. O avanço das técnicas na aplicação da atropelastes de joelho, tornou a técnica de uso freqüente e crescente na última década, condicionando o seu sucesso a diversos fatores: o desenho dos componentes, a qualidade dos materiais disponíveis, a técnica de fabricação, aplicação adequada a cada paciente, como também o desenvolvimento da técnica cirúrgica. Metodologia: Trata-se de uma revisão narrativa da literatura. Resultados: Um grande instrumento é a navegação de artroplastia total de joelho, sendo este um instrumento que facilita a execução operatória, tendo também a capacidade de auxiliar o cirurgião na tomada de decisões intraoperatorias, possibilitando a simulação de situações antes de realizar-las. Tem grande utilização no campo do ensino, pois possibilita ver imediatamente o efeito de cada ação realizada durante operação, sendo possível que o navegador dispensa a necessidade de habilitação do cirurgião ou diminua o valor da sua experiência, pois o navegador não aponta caminhos ou tomar decisões pelo cirurgião, realizando apenas o auxílio na obtenção e aferição dos resultados planejados para cada ação. Esta atividade potencializa o conhecimento e as experiências vividas pelo cirurgião pois fornece grandes parâmetros com alta precisão e objetivos em tempo real na operação. Conclusão: Uso da robótica na cirurgia ortopédica deve ser vista como instrumento a disposição do Cirurgião, evidenciando o seu conhecimento e jamais como sistema independente autônomo que possa compensar uma eventual deficiência de experiência ou conhecimento do mesmo.
