Maternal care: a health prevention strategy to develop healthy people

Resumo: After the first year of experience of the Center for Supporting the Family Health (CSFM), an analysis of the main concerns or symptoms was made. In the children’s population, it was observed that there are difficulties related to language, learning, behavior, and bonding. In adults, difficulties related to autonomy to deal with health/illness, body and conflict resolution were observed. Based on guidelines of Primary Care, an intervention was developed to deal with complaints from the weak links that constitute parental relationships, which lead to consequences for the development of subjects. To this end, we created the motherly care group, which is the activity of the child care that provides necessary care and emotional support for a healthy neurodevelopment. This is an educational-therapeutic group. It was observed a change of the participating mothers in relation to education and placement of boundaries, elaboration of family conflicts, reflection on their relationship with their parents, and improvement on listening and binding. However, in the babies, there was a better binding and acceptance of maternal touch and limits, greater sense of peace, and improvement on neuropsychomotor skills. The challenge is to deal with mothering care with subjects that were little or that were not held at all by their mothers. These are vulnerable subjects, little autonomous, and with great difficulty in establishing relationships guided by trust and security. At many moments, mothers are the ones who feel the holding action, redefining their first bonds, recovering the act of playing and the fantasy. It is believed that the mother-subject change can be achieved through the transformation of the mother-infant relationship, which will be decisive in shaping the subject-baby and allowing the redefinition of family relationships, producing health.

Después de la experiencia del primer año del Núcleo de Apoyo a la Salud de la Familia (NASF), se hizo un análisis de las principales preocupaciones o síntomas. En la población pediátrica, se pudo observar dificultades asociadas con el lenguaje, aprendizaje, comportamiento y vínculos. En la población adulta, las dificultades relacionadas con la autonomía de la salud/enfermedad, el cuerpo y la resolución de conflictos. Con base en los lineamientos de la Atención Primaria, hemos creado una intervención para tratar las quejas de los vínculos débiles que constituyen las relaciones de los padres, con consecuencias para el desarrollo y capacitación de los sujetos. Con este fin, hemos creado el grupo de la maternidad, que es la actividad que brinda al niño la atención necesaria o apoyo emocional para un desarrollo neurológico saludable. Es un grupo educativo-terapéutico. Se observó un cambio en el posicionamiento de las madres participantes en relación a la educación y la imposición de límites, la preparación de los conflictos familiares, reflexión sobre la relación con los padres, la mejora de la escucha y los vínculos. Ya en los bebés, hubo una mejor unión, y de la aceptación del toque maternal y los límites. Hubo más tranquilidad y mejoría de las habilidades neuropsicomotoras. El reto es hacer frente a la maternidad con los sujetos que eran poco maternados, por lo tanto vulnerables, poco autónomos, con dificultad para establecer relaciones basadas en la confianza/seguridad. En muchos casos, son las madres las que son maternadas, las que vuelven a redefinir sus primeros vínculos, recuperándose los juegos y fantasías. Se cree que el cambio del sujeto-madre se pueda dar a través de la transformación de la relación madre-hijo, que será fundamental en la formación del sujeto-bebé y que permitirá la redefinición de las relaciones familiares, produciéndose así salud.Após o primeiro ano de experiência do Núcleo de Apoio à Saúde da Família (NASF), realizou-se uma análise das principais demandas ou sintomas. Na população infantil, observaram-se dificuldades relacionadas à linguagem, aprendizagem, comportamento e vinculação. Na população adulta, foram encontradas dificuldades relacionadas à autonomia no processo saúde/doença, corpo e resolução de conflitos. Com base nas diretrizes da Atenção Básica, elaborou-se uma intervenção para lidar com as queixas provenientes dos vínculos frágeis que constituem as relações parentais, acarretando consequências para o desenvolvimento ou formação dos sujeitos. Para tanto, criou-se o grupo de Maternagem, que se trata da atividade de cuidar que propicia à criança atenção necessária e suporte emocional para um desenvolvimento neuropsicomotor saudável. Trata-se de um grupo terapêutico-educativo. Em relação às mães, observou-se mudança no posicionamento em relação à educação e colocação de limites, elaboração de conflitos familiares, reflexão sobre sua relação com seus pais, aprimoramento da escuta e vinculação. Nos bebês, observou-se melhora na vinculação, na aceitação do toque materno e nos limites, maior tranquilidade e aprimoramento das habilidades neuropsicomotoras. O desafio é tratar de maternagem com sujeitos que foram pouco maternados, portanto, vulneráveis, pouco autônomos, com dificuldade de estabelecer relações pautadas na confiança/segurança. Em muitos momentos, as mães é que são maternadas, ressignificando seus primeiros vínculos, recuperando o brincar e a fantasia.
