Efeito de um programa de comunicação alternativa para a capacitação de mães de crianças com paralisia cerebral não verbal

Resumo: Communication is an important resource for child development, because through it the children acquire communication skills and relate to people. Children with severe communication disorders require a rehabilitation program directed to the orientation and manipulation of the user and the family. In this context, it is essential the participation of the person responsible for the child within the family context in the process of choose, development and adaptation of the resource, since parents know better wishes, the center of interest and the routine of their children. The partnership between parents and health professionals has the purpose to make the family an active and cooperative member during this process. The objective of this study was to develop, to implement and to evaluate an Individualized Program of Augmentative and Alternative Communication for parents of children with Cerebral Palsy. The research took place in the Family and School Unity at Federal University of São Carlos, located in São Paulo State, Brazil. The study included 3 children with cerebral palsy and their mothers. In this study was it was used ten instruments; 5 of them were constructed specifically for this study: two protocols about Characterization of the participants; one Interview outline with the objective to identify communication skills already used by the child through the report of the mothers; Protocol for analyze the baseline and intervention with objective to describe in details the video of the baseline and intervention, an outline to analyze mothers perceptions about program effectiveness, Motor Function Measure Classification System with the objective to classify children motor function movements in sitting and walking, Brazil Criterion Questionnaire was used to evaluate and categorize the possession of consumer durables and education level of household head and the Protocol for the Selection of the figures in order to select items from children interest reported by mothers, which are related to the theme of self-care, productivity and leisure. In order to verify the effects of the intervention it used Multiple Baseline Design cross subjects including two phases: baseline and intervention. The results were described in three studies. The study I reveals data on the communication skills of children before and after the intervention. The data showed existing communicative skills in children, and a low input repertoire and acquire new skills through the implementation of alternative communication. The study II data revealed Intervention Process, mothers and children before the intervention process lacked a repertoire of meaningful input before the issue of alternative communication. With the intervention, the repertoire of the mothers was significant and there was an excellent performance in using the same communication capabilities as well as an alternative expressive performance by children in communicating their desires through the figures. Study III showed the mothers' perceptions about the relevance of the use of communication resources to get your kids to express themselves in situations of everyday life. Thus, the premise that the deployment of communications capabilities supplementary and/ or alternative through a partnership with mothers of children with cerebral palsy do not favor the expansion of verbal interaction and acquire new communication skills was confirmed. Considering the results presented above, the above research hypothesis was tested and confirmed. Thus, it is expected that future studies can be developed from the perspective of socializing program in clinical and educational context of daily life of children with cerebral palsy nonverbal.

Crianças com severos distúrbios na comunicação necessitam de um programa de reabilitação direcionado à orientação e instrumentalização do usuário e família. Neste contexto, é essencial a participação da família no processo de escolha, elaboração e confecção do recurso, pois os pais conhecem os desejos, centro de interesse e a rotina de seus filhos. A parceria entre pais e os profissionais da saúde e educação tem a finalidade de fazer com que a família seja um membro ativo e cooperador durante o processo. O presente estudo teve por objetivo elaborar, implementar e avaliar um Programa Individualizado de Comunicação Suplementar e/ou Alternativa para mães de crianças com Paralisia Cerebral, capacitar as mães de crianças com paralisia cerebral não verbais a utilizarem os recursos de comunicação alternativa, verificar a eficácia do programa e a percepção das mães sobre a relevância do uso dos recursos de comunicação para seu filho. A pesquisa foi realizada em uma unidade de saúde, de uma cidade do interior do estado de São Paulo, vinculada a uma Universidade Pública. Participaram da pesquisa três crianças com Paralisia Cerebral e suas mães. Para a coleta de dados foram utilizados 10 instrumentos: dois Protocolos de Caracterização dos participantes, um Roteiro de Entrevista com objetivo de identificar as habilidades comunicativas já utilizada pela criança, Protocolo para Análise das sessões de Linha de base, intervenção e follow up com objetivo de descrever minuciosamente as filmagens de linha de base e intervenção, Roteiro para Analisar a Percepção das mães com objetivo de analisar a eficácia do programa individualizado de comunicação alternativa, Questionário de Validade Social com objetivo de saber o grau de satisfação e opinião das mães quanto a importância dos procedimentos realizados durante o desenvolvimento do programa, a Escala Gross Motor Function Measure Classification System a fim de classificar a função motora dos movimentos de sentar e caminhar das crianças, Questionário Critério Brasil com objetivo de avaliar e categorizar a posse de bens de consumo duráveis e o grau de escolaridade do chefe de família e o Protocolo para Seleção das figuras com o propósito de selecionar os itens do centro de interesse das crianças e o Protocolo de Observação Comportamental, o qual objetivou analisar as habilidades comunicativas das crianças antes e após a intervenção. Para responder aos objetivos, a pesquisa utilizou o delineamento de linha de base múltipla. Os resultados foram descritos em três estudos. O estudo I revela dados sobre as habilidades comunicativas das crianças antes e após a intervenção. Os dados demonstraram as habilidades comunicativas já existentes nas crianças, ou seja, um baixo repertório de entrada e a aquisição de novas habilidades a partir da implementação da comunicação alternativa. O estudo II revelou os dados do Processo de Intervenção, no qual as mães e as crianças antes do processo de intervenção não possuíam um repertório de entrada expressivo perante a temática da comunicação alternativa. Com a intervenção, o repertório das mães mostrou-se expressivo e houve um excelente desempenho das mesmas em utilizar os recursos de comunicação alternativa bem como um desempenho expressivo por parte das crianças em comunicar seus desejos por meio das figuras. O estudo III mostrou a percepção das mães sobre a relevância do uso dos recursos de comunicação para seus filhos conseguirem se expressar diante de situações do cotidiano. Dessa forma, a premissa que a implementação de recursos de comunicação suplementar e/ou alternativa por meio da parceria com mães de crianças com paralisia cerebral não verbais favorecia a ampliação da interação e aquisição de novas habilidades comunicativas foi confirmada.
