Saúde mental e atenção psicossocial em desastres: comportamentos profissionais do psicólogo intervindo na resposta

Resumo: Response is a stage of disaster management contemplated in the National Policy for Protection and Civil Defence (in Portuguese, PNPDC). It encompasses actions during and immediately after the event as means to provide aid and assist those affected, as well as return essential services. Mental Health and Psychosocial Attention (SMAPS) care to all individuals affected is recommended in the literature. However, in Brazil, we identified the need for training of public healthcare providers for such action. Some of the skills for the field were mapped and may be useful in training programmes. Although criticised, skills and content models are widespread for professional training in different areas. In Brazil, another possibility is the model of professional behaviour. A teaching technology that uses this concept is the Programming of Conditions for Behaviour Development (in Portuguese, PCDC). PCDC's concept of behaviour is defined as the relationship between what the organism does, the context in which they do it, and what follows its occurrence. In order to develop a teaching program, it is necessary to analyse and break down target behaviours for a task. Clarity, objectivity, precision and conciseness of teaching objectives have been adopted as criteria. False teaching goals are criticized. Based on the model of professional behaviours, the objective of this study is to characterize the general class of behaviours “acting directly in mental health and psychosocial care of those affected in disaster responses”. The classes that compose it were organized based on a model of general classes of professional behaviour for direct intervention in different fields of action, and in the four levels of action in Mental Health and Psychosocial Support available in the Pyramid of the Interagency Standing Committee (in Portuguese, IASC). As a source of information, a set of possible psychologists' skills for crisis intervention and prevention of Post-Traumatic Stress in disaster response was selected, which have been mapped in a previous study. As a result of a 10-step procedure, 72 classes of behaviours were discovered and organized into range levels. The six most comprehensive classes were: (1) Characterizing the need for intervention in relation to the behaviours of those affected by disasters in an SMAPS care context; (2) Designing interventions; (3) Implementing interventions; (4) Evaluating interventions; (5) Improving interventions; (6) Communicating findings. The current PCDC procedure allowed for greater clarity on psychologists’ possible goals while caring for those affected in acute distress, which can be adopted in future training. Here we placed greater emphasis on characterizing the class “Applying Psychological First Aid techniques”, but the procedure may be useful in discovering other classes of behaviours that make up SMAPS care in the response. This technology is also a possibility for discovering classes that make up the implementation in other phases of comprehensive risk and disaster management, as described in the PNPDC, such as prevention, mitigation, preparation, and recovery.

É composta por ações realizadas durante ou logo após o evento, de forma a socorrer e assistir os afetados e retornar os serviços essenciais. Os cuidados em Saúde Mental e Atenção Psicossocial (SMAPS) a todos os afetados são recomendados pela literatura. Entretanto, no Brasil, foram identificadas necessidades de capacitações junto aos profissionais da rede pública de saúde para essa atuação. Algumas competências para o campo foram mapeadas e podem ser úteis em treinamentos. Os modelos das competências, e dos conteúdos são difundidos para formação profissional em diferentes áreas, porém criticados. No Brasil, outra possibilidade é o modelo dos comportamentos profissionais. Uma tecnologia de ensino que utiliza esse último conceito é a Programação de Condições para Desenvolvimento de Comportamentos (PCDC). O conceito de comportamento da PCDC é definido como a relação entre o que o organismo faz, o contexto em que o faz, e o que decorre do fazer. Para desenvolver um programa de ensino é necessário analisar e decompor os Comportamentos-objetivos para uma atuação. Têm sido adotados como critérios a clareza, a objetividade, a precisão e a concisão dos objetivos, e criticados os falsos objetivos de ensino. A partir do modelo dos comportamentos profissionais, o objetivo desta pesquisa é caracterizar a classe geral de comportamentos “intervir diretamente no cuidado em saúde mental e atenção psicossocial de afetados na resposta a desastres”. As classes que a compõe foram organizadas a partir de um modelo de classes gerais de comportamentos profissionais para intervenção direta em diferentes campos de atuação, e nos quatro níveis de ação em Saúde mental e apoio Psicossocial previstos na Pirâmide do Comitê Permanente Interagências (IASC). Como fonte de informações foi selecionado um conjunto de possíveis competências do psicólogo para intervenção em crise e prevenção de Estresse Pós-Traumático na resposta a desastres, mapeadas em estudo anterior. Como resultado de um procedimento de 10 etapas, foram descobertas e organizadas 72 classes de comportamentos em níveis de abrangência. As seis classes mais abrangentes foram: (1) Caracterizar a necessidade de intervenção em relação a comportamentos de afetados por desastres em um contexto de cuidados em SMAPS; (2) Projetar intervenções; (3) Executar intervenções; (4) Avaliar intervenções; (5) Aperfeiçoar intervenções; (6) Comunicar descobertas feitas. O procedimento da PCDC permitiu maior clareza sobre os possíveis objetivos do psicólogo no cuidado ao afetado em sofrimento agudo, que podem ser adotados em futuras capacitações. Nesse trabalho, houve maior ênfase na caracterização da classe “Aplicar a técnica dos Primeiros Socorros Psicológicos”, mas o procedimento poderá ser útil na descoberta de outras classes de comportamentos que compõem os cuidados em SMAPS na resposta.
