Variables related to the repetition of a suicide attempt: a systematic literature review

Resumo: Systematic review of studies published in the past 20 years to identify variables associated with repeated suicide attempts. A search for articles published since 2000 on the PubMed and Virtual Health Library portals was conducted, using the words: suicide attempt, suicide, repetition and recurrence. Information regarding date of publication, population, place, means of collection used and associated variables were extracted. The data were later organized into categories of analysis. In total 23 studies were selected and reviewed. There was more correspondence for repetition in people who had already attempted suicide. Mental disorder and substance abuse appeared next. Results pointed to suicide attempt as the variable most associated with new suicide attempts and suicide. Based on the findings, it is suggested that preventive actions can have a greater impact if initiated before the presentation of the first suicide attempt. In populations with a history of suicide attempt or multiple attempts, it is argued that interventions focused on adherence and risk monitoring from hospital discharge to outpatient follow-up can be critical to prevent new attempts and consequently the suicide rate.

Se realizó una búsqueda de artículos publicados desde 2000 en los portales PubMed y Biblioteca Virtual da Saúde, utilizando las palabras: intento de suicidio, suicidio, repetición y recurrencia. Se extrajo información sobre fecha de publicación, población, ubicación, medio de recolección utilizado y variables asociadas. Posteriormente, los datos se organizaron en categorías de análisis. En total, se seleccionaron y revisaron 23 estudios. Hubo más correspondencia por repetición en personas que ya habían intentado suicidarse. A continuación aparecieron el trastorno mental y el abuso de sustancias. Los resultados apuntaron al intento de suicidio como la variable más asociada a nuevos intentos de suicidio y al suicidio. Con base en los hallazgos, se sugiere que las acciones preventivas pueden tener un mayor impacto si se inician antes de la presentación del primer intento de suicidio. En poblaciones con antecedentes de intento de suicidio o múltiples intentos, se argumenta que las intervenciones centradas en la adherencia y la monitorización del riesgo desde el alta hospitalaria hasta el seguimiento ambulatorio pueden ser críticas para prevenir nuevos intentos y, en consecuencia, la tasa de suicidio.Revisão sistemática de estudos publicados nos últimos 20 anos para identificar variáveis associadas à repetição de tentativas de suicídio.  Foi conduzida uma busca de artigos publicados a partir de 2000 nos portais PubMed e Biblioteca Virtual da Saúde, por meio das palavras: tentativa de suicídio, suicídio, repetição e recorrência.  Foram extraídas informações referente a data de publicação, população, local, meios de coleta utilizados e variáveis associadas. Os dados foram posteriormente organizados em categorias de análise. No total 23 estudos foram selecionados e revisados. Houve maior correspondência para repetição em pessoas que já haviam tentado um suicídio. Transtorno mental e o uso abusivo de substâncias, apareceram em seguida. Resultados apontaram a tentativa de suicídio como a variável mais associada a novas tentativas de suicídio e suicídio. Sugere-se com base nos achados que ações preventivas podem ter maior impacto se iniciadas antes da apresentação da primeira tentativa de suicídio.
