Resumo: Objectives: To describe the development of an instrument for clinical and psychosocial assessment of patients, after discharge from the ICU, to submit it to the content validation process and verify its applicability in the target population. Methods: Methodological development study based on Pasquali's content validation model (1998), carried out in three stages: construction of the items, evaluation of the instrument by a panel of specialists in the areas such as nursing, physiotherapy, nutrition, psychology and social work and, finally, the analysis of the evaluator’s responses using the Content Validity Index (CVI) and the Inter-Evaluator Agreement Index (IRA). After validation, the instrument was applied in the target population. Results: Most evaluators have a doctorate degree (67%), with an average experience of 17.6 ± 8.38 years, most of them working in the teaching and assistance field. There was a little disagreement among experts in analyzing the issues according to the construct's domains. Most items were positively evaluated, receiving a score above 80%, according to the instrument's validation criteria. Items that did not reach this score were reformulated and / or excluded. The final version of the survey was applied to a group of patients after discharge from the ICU, and proved to be satisfactorily understandable, indicating semantic validity. Conclusion: The instrument obtained a positive global assessment related to the proposed items, being considered valid regarding content and applicability in the patient population, six months after discharge from the ICU.
