CLINICAL CHARACTERIZATION OF WOMEN SUBMITTED TO OBSTETRIC HYSTERECTOMY AS A CONSEQUENCE OF POSTPARTUM HEMORRHAGE

Resumo: Objective: To describe the main clinical characteristics of women submitted to obstetric hysterectomy due to postpartum hemorrhage. Methods: descriptive-exploratory study, with a quantitative approach and documentary character. The following inclusion criteria were adopted: medical records of women who gave birth via vaginal delivery or cesarean section and aged 18 years or over; who have undergone an obstetric hysterectomy due to postpartum hemorrhage; and submitted to the surgical procedure from January 2018 to June 2022. Therefore, only 15 medical records met the criteria. Results: cesarean section was the main mode of delivery (93.3%) and fetal distress was the main indication for this route (21.4%). Postpartum hemorrhage was classified as primary (93.3%) and had uterine atony as the main cause (86.6%). Subtotal hysterectomy was the most used surgical technique (93.3%) and the mean time from fetal expulsion to obstetric hysterectomy was 6.4 hours. Conclusion: This study made it possible to highlight the importance of the topic addressed, since postpartum hemorrhage is one of the main causes of maternal mortality worldwide. Therefore, knowing the clinical profile of these women is essential to provide quality care, in addition to offering subsidies for protocols and future research.

Métodos: estudio descriptivo-exploratorio, con abordaje cuantitativo y carácter documental. Fueron adoptados los siguientes criterios de inclusión: prontuario de mujeres que dieron la luz por parto vaginal o cesárea y con edad igual o superior a 18 años; que se han sometido a una histerectomía obstétrica por hemorragia posparto; y se sometieron al procedimiento quirúrgico de enero de 2018 a junio de 2022. Por lo tanto, solo 15 historias clínicas cumplieron con los criterios. Resultados: la cesárea fue la principal vía de parto (93,3%) y el sufrimiento fetal la principal indicación por esta vía (21,4%). La hemorragia posparto se clasificó como primaria (93,3%) y tuvo como principal causa la atonía uterina (86,6%). La histerectomía subtotal fue la técnica quirúrgica más utilizada (93,3%) y el tiempo medio desde la expulsión fetal hasta la histerectomía obstétrica fue de 6,4 horas. Conclusión: Este estudio destacó la importancia del tema abordado, ya que la hemorragia posparto sigue siendo una de las principales causas de mortalidad materna a nivel mundial. Por lo tanto, conocer el perfil clínico de estas mujeres es fundamental para brindar una atención de calidad, además de ofrecer subsidios para protocolos y futuras investigaciones.Objetivo: Descrever as principais características clínica de mulheres submetidas à histerectomia obstétrica por consequência da hemorragia pós-parto. Métodos: estudo descritivo-exploratório, de abordagem quantitativa e caráter documental. Adotou-se como critérios de inclusão, prontuários de mulheres que pariram via parto vaginal ou cesariana, com idade igual ou acima de 18 anos; que tenham sido submetidas a histerectomia obstétrica em decorrência da hemorragia pós-parto; e submetidas ao procedimento cirúrgico de janeiro de 2018 a junho de 2022. Diante disso, 15 prontuários atenderam aos critérios. Resultados: a cesariana foi a principal via de parto (93,3%) e o sofrimento fetal a maior indicação dessa via (21,4%). A hemorragia pós-parto foi classificada como primária (93,3%) e teve a atonia uterina como maior causa (86,6%). A histerectomia subtotal foi a técnica cirúrgica mais utilizada (93,3%) e o tempo médio da expulsão do feto até a realização da histerectomia obstétrica ficou em 6,4 horas. Conclusão: Este estudo permitiu evidenciar a importância do tema tratado, uma vez que a hemorragia pós-parto é uma das principais causas de mortalidade materna em todo mundo.
