ESSENTIAL ELEMENTS FOR THERAPEUTIC ADHERENCE IN PATIENTS WITH HEART FAILURE AT HOSPITAL DISCHARGE: INTEGRATIVE REVIEW

Resumo: Objective: to identify through scientific literature which elements should be inserted in an educational technology to assist in the therapeutic adherence of patients with Heart Failure (HF) at hospital discharge. Method: integrative literature review carried out during the month of January 2021. For the search, an association of 4 subject descriptors from the Medical Subject Headings (MeSH) of the National Library of Medicine (NLM) and National Institutes of Health (NIH) was used, being two controlled (“Heart Failure” and “Patient Discharge”) and two uncontrolled (“Adherence and Compliance to Treatment” and “Educational Technology”). And a triplet with the boolean operator “AND” to associate the descriptors. Results: there were studies developed by nurses, highlighting aspects that contribute to greater patient adherence. Therefore, the resources for therapeutic adherence in extracted items were categorized in relation to the essential elements for therapeutic adherence, some categories were extracted from the studies: pharmacological treatment, non-pharmacological treatment, clinical guidelines on HF, communication with patients and families, patient autonomy for care. Final Considerations: it was possible to identify the construction of educational technology for patients with HF who are at hospital discharge, should be based on different dimensions of care.

Método: revisión integrativa de la literatura realizada durante el mes de enero de 2021. Para la búsqueda se utilizó una asociación de 4 descriptores de materias de los Medical Subject Headings (MeSH) de la Biblioteca Nacional de Medicina (NLM) y los Institutos Nacionales de Salud (NIH), siendo dos controlados (“Insuficiencia cardíaca” y “Alta del paciente”) y dos no controlados (“Cumplimiento y adherencia al tratamiento” y “Tecnología educativa”). Como estrategia de búsqueda se realizó el cruce por parejas y un trío con el operador booleano “AND” para asociar el descriptores. Resultados: hubo un predominio de estudios desarrollados por enfermeros, mostrando aspectos que contribuyen para una mayor adherencia de los pacientes con IC. Por lo tanto, los elementos esenciales para la adherencia terapéutica extraídos de los artículos se categorizaron en: tratamiento farmacológico, tratamiento no farmacológico, guías clínicas sobre IC, comunicación con pacientes y familiares, autonomía del paciente para el cuidado. Consideraciones finales: fue posible identificar que la construcción de una tecnología educativa para pacientes con IC que reciben alta hospitalaria debe basarse en diferentes dimensiones del cuidado.Objetivo: identificar por meio da literatura científica quais elementos a serem inseridos em uma tecnologia educacional para auxiliar na adesão terapêutica de pacientes com Insuficiência Cardíaca (IC) na alta hospitalar. Método: revisão integrativa da literatura realizada durante o mês de janeiro de 2021. Foram utilizados para a busca uma associação de 4 descritores de assunto do Medical Subject Headings (MeSH) da National Library of Medicine (NLM) e National Institutes of Health (NIH), sendo dois controlados (“Heart Failure” e “Patient Discharge") e dois não controlados (“Treatment Adherence and Compliance” e “Educational Technology”). Realizou-se como estratégia de busca o cruzamento aos pares e um trio com o operador booleano “AND” para associação dos descritores. Resultados: houve prevalência de estudos desenvolvidos por enfermeiros, evidenciando aspectos que contribuem para a maior adesão de pacientes com IC. Para tanto, os elementos essenciais para a adesão terapêutica extraído dos artigos foram categorizados em: tratamento farmacológico, tratamento não-farmacológico, orientações clínicas sobre a IC, comunicação com os pacientes e familiares, autonomia do paciente para o cuidado.
