Evaluation of primary health care: comparison between organizational models

Resumo: | Primary Health Care (APS) refers to a set of practices that aims to meet the health needs of the individual and the community. At this level of care, certain attributes must be developed to ensure the quality of services: First Contact, Longitudinality, Integrality, Coordination, Family Focus, Community Orientation, and Cultural Competence. The objective of this study was to identify, evaluate, and compare the attributes of Primary Health Care considering two organizational models. We did an exploratory, cross-descriptive research, applying the Primary Care Assessment Tool Brazil Instrument, professional version, to 70 doctors and 33 nurses working in primary care in a city in the state of São Paulo, Brazil. Descriptive and inferential statistics were used. Traditional primary care showed average scores above 6.6 in the attributes of coordination, longitudinality, comprehensiveness − services provided and family guidance. The attributes that received a low score were comprehensiveness − available services, community guidance, and first contact access, the latter being the only attribute with an average score below 6.6 in the Family Health Strategy. A difference was identified between the models, with better results in the Family Health Strategy compared to traditional primary care, in the essential, derived and general scores. It was concluded that the Family Health Strategy presented better results in the evaluated attributes, representing greater quality. Therefore, the expansion of coverage by this model is presented as a strategy to strengthen this level of assistance. The applicability of the study arises from the strategic sharing of results for professionals and managers, aiming at the joint construction of coping guidelines: awareness of the priority agenda for APS, insertion of the Reflection of the Practice as a Permanent Education tool for the teams, strengthening of the Regional Health (DRS) partnership with the university. |

En este nivel de atención, se deben desarrollar ciertos atributos para garantizar la calidad de los servicios: Primer Contacto, Longitudinalidad, Integralidad, Coordinación, Enfoque Familiar, Orientación Comunitaria y Competencia Cultural. El objetivo de este estudio fue identificar, evaluar y comparar estes atributos, considerando dos modelos organizativos. Se realizó una investigación exploratoria, descriptiva cruzada, aplicando el Primary Care Assessment Tool Brasil a 70 médicos y 33 enfermeros que laboran en atención primaria en una ciudad de São Paulo, Brasil. Se utilizó estadística descriptiva e inferencial. La atención primaria tradicional presentó puntuaciones medias superiores a 6,6 en los atributos de coordinación, longitudinalidad, integralidad − servicios prestados y orientación familiar. Los atributos que recibieron una puntuación baja fueron la integralidad − servicios disponibles, orientación comunitaria y acceso de primer contacto, siendo este el único atributo con una puntuación media inferior a 6,6 en la Estrategia Salud de la Familia. Se identificó una diferencia entre los modelos, con mejores resultados en la Estrategia Salud de la Familia frente a la atención primaria tradicional, en los puntajes esencial, derivado y general. La Estrategia Salud de la Familia presentó mejores resultados en los atributos evaluados, lo que representa una mayor calidad. La ampliación de la cobertura por este modelo se presenta como una estrategia para fortalecer este nivel de asistencia. La aplicabilidad del estudio surge de la puesta en común estratégica de resultados con miras a la construcción conjunta de lineamientos de afrontamiento: conocimiento de la agenda prioritaria de la APS, inserción de la Reflexión de la Práctica como herramienta de Educación Permanente para los equipos, fortalecimiento de la Asociación Regional de Salud (DRS) con la universidad.A Atenção Primária à Saúde (APS) refere-se a um conjunto de práticas que objetiva atender às necessidades de saúde individuais e coletivas. O objetivo deste estudo foi identificar, avaliar e comparar os atributos da Atenção Primária à Saúde, considerando dois modelos organizativos. Realizou-se pesquisa exploratória, transversal-descritiva, aplicando o Instrumento Primary Care Assessment Tool Brasil, versão profissional, a 70 médicos e 33 enfermeiros atuantes na atenção básica de um município do Estado de São Paulo, Brasil. Utilizou-se estatística descritiva e inferencial. A atenção básica tradicional apresentou escores médios acima de 6,6 nos atributos coordenação, longitudinalidade, integralidade – serviços prestados e orientação familiar. Os atributos que receberam baixo escore foram integralidade – serviços disponíveis, orientação comunitária e acesso de primeiro contato, sendo este último o único atributo com escore médio abaixo de 6,6 na Estratégia Saúde da Família. Identificou-se diferença entre os modelos, com resultados melhores na Estratégia Saúde da Família em comparação à atenção básica tradicional, nos escores essencial, derivado e geral. Concluiu-se que a Estratégia Saúde da Família apresentou melhores resultados nos atributos avaliados, representando maior qualidade. Portanto, a ampliação de cobertura por este modelo apresenta-se como estratégia de fortalecimento deste nível assistencial.
