Diagnostic approach of the patient with facial pain: review of the literature

Resumo: The aim of the study is to review and describe the main available evidence related to the diagnostic approach to facial pain, especially in the primary care setting, and to provide a tool that synthesizes the findings.This article is a literature review. A search was carried out in databases such as PubMed, SciELO, LILACS, Google Academic, and Scopus. Keywords "facial pain", "facial neuralgia", "trigeminal nerve disease", and "diagnosis" were used. It was found that facial pain should be approached syndromatically and etiologically, identifying aspects of pain suggestive of central or peripheral neurological alteration, and conducting a complete clinical and paraclinical study to identify the etiology. We conclude that facial pain requires a comprehensive approach and should be a central topic in teaching programs in medicine and dentistry.

El objetivo del estudio es revisar y describir la principal evidencia disponible relacionada con el enfoque diagnóstico del dolor facial, especialmente en el entorno de atención primaria, y brindar una herramienta que sintetice los hallazgos. Se realizó una revisión de la literatura, mediante una búsqueda en bases de datos como PubMed, SciELO, LILACS, Google Académico, y Scopus. Se usaron como palabras clave “facial pain”, “facial neuralgia”, “trigeminal nerve disease”, y “diagnosis”. Se encontró que el dolor facial se debe enfocar de forma sindromática y etiológica, identificando aspectos del dolor sugestivos de alteración neurológica central o periférica, y realizando un estudio completo clínico y paraclínico que permita identificar la etiología. Concluimos que el dolor facial requiere un abordaje integral y debe ser un tópico central en los programas de enseñanza en medicina y odontología.O objetivo do estudo é revisar e descrever as principais evidências disponíveis relacionadas à abordagem diagnóstica da dor facial, especialmente na atenção primária, e fornecer uma ferramenta que sintetize os achados. Foi realizada uma revisão da literatura por meio de buscas em bases de dados PubMed, SciELO, LILACS, Google Scholar e Scopus. Foram utilizadas as palavras-chave “facial pain”, “facial neuralgia”, “trigeminal nerve disease”, e “diagnosis”. Encontrou-se que a dor facial deve ser abordada sindromaticamente e etiologicamente, identificando aspectos da dor sugestivos de alteração neurológica central ou periférica e realizando um estudo clínico e paraclínico completo para identificação da etiologia.
