O cuidado profissional ao idoso Kaigang na terra indígena Faxinal, Paraná, Brasil

Resumo: The aging population has increased the demand in the need for knowledge about the health care The prolongation of life is an aspiration of any society, however, it can only be considered a real achievement if does not only extend survival, but provides a full life with good quality. Numerous studies have been performed in order to know how the service performs health care for this population, however nothing has been seen in relation to the elderly indigenous. Given the above, this work proposes to understand and analyze the care practices of health professionals who assist elderly Kaingang and their weaknesses and strengths to help and improve this assistance. This is a study described in a qualitative ethnographic, conducted by ten professionals working in primary health care of Indigenous in Faxinal (TIF), Paraná, Brazil. Data was collected from November 2010 to February 2012, through eight periods of fieldwork in TIF staying approximately seven days each period. The data collection techniques, participant observation and interviews, were used along with field journal as a source of data recording. The analysis of the findings is given through the theoretical framework of Madeleine Leininger. The results revealed that professional care for the elderly is permeated by indigenous scientific and traditional knowledge. The scientists reports the medication administration, conducting fogging, curative and laboratory tests. While the traditional knowledge was apparent the use of teas and herbal drugs. It also showed how carefully the performance of the personal hygiene in the Basic Health Care and preoccupation with alimentation demonstrations of affection by health professionals to the elderly Kaingang. For those who care practices could be accomplished, it was found that healthcare professionals adopt strategies that contribute to both the elderly access to Indian Health Service, and for staying in this care available for them. Although health professionals conduct a careful that values traditional practices in geriatric healthcare Kaingang, it was observed that health actions targeting this part of the population still is focused exclusively on the subject, with an individualized vision. This practice was argued by health professionals due to several weaknesses that permeate the realization of indigenous elderly care, and these limitations encompassed in Kaingang aspects of culture and organization of local health service. The conclusion that there´s a need for restructuring assistance with improving the planning of action and working conditions of health professionals. The results of this study provide information for nurses who work or will work in Indigenous health by offering them subsidies to expand the possibilities of caring in this space of new knowledge and practices, contributing to the improvement of health care for the elderly indigenous.

Inúmeras pesquisas vêm sendo realizadas no intuito de conhecer como o serviço de saúde realiza a assistência para esta parcela populacional, entretanto nada se tem visto no que se refere aos idosos indígenas. Diante do exposto, este trabalho propõe conhecer e analisar as práticas de cuidado dos profissionais de saúde que assistem idosos Kaingang e as respectivas fragilidades e potencialidades para a realização desta assistência. Trata-se de um estudo descrito de abordagem qualitativa apoiada na etnográfica, realizado com dez profissionais que atuam na atenção primária à saúde da Terra Indígena Faxinal (TIF), Paraná, Brasil. Os dados foram coletados no período de novembro de 2010 a Fevereiro de 2012, por meio de oito momentos de trabalho de campo na TIF com permanência de aproximadamente sete dias cada período. Utilizou como técnicas de coleta de dados a observação participante e entrevistas, e o diário de campo como fonte de registro dos dados. A análise dos achados se deu por intermédio do referencial teórico de Madeleine Leininger. Os resultados revelaram que o cuidado profissional aos idosos indígenas é permeado por saberes científico e saberes tradicional. Os científicos reportam a administração de medicamentos, realização de nebulização, curativo e exames laboratoriais. Quanto aos saberes tradicionais foram evidentes a realização de chás e utilização de fitoterápicos. Evidenciou-se também como cuidado, a realização do banho na Unidade Básica de Saúde, cuidados com a alimentação e manifestações de afeto pelos profissionais de saúde aos idosos Kaingang. Para que tais práticas de cuidado pudessem ser efetivadas, constatou-se que os profissionais de saúde adotam estratégias que contribuem tanto para o acesso do idoso indígena ao serviço de saúde, quanto para a permanência deste no cuidado disponibilizado por eles. Embora os profissionais de saúde realizem um cuidado que valorize as práticas tradicionais na assistência ao idoso Kaingang, observou-se que as ações em saúde voltadas para esta parcela populacional ainda encontra-se centrada apenas no sujeito, com uma visão individualizada. Tal prática foi argumentada pelos profissionais de saúde devido às diversas fragilidades que permeiam a concretização do cuidado ao idoso indígena, sendo estas limitações englobadas nos aspectos da cultura Kaingang e na organização do serviço de saúde local. Conclui-se a necessidade de reestruturação da assistência prestada com melhoria no planejamento das ações e nas condições de trabalho dos profissionais de saúde.
