EFEITO DE DOIS ANTI-INFLAMATÓRIOS E UM ANTIOXIDANTE NA PREVENÇÃO DA SENSIBILIDADE CAUSADA PELO CLAREAMENTO DENTAL

Resumo: The tooth bleaching has a long history of tooth sensitivity (TS). This study aimed to determine the effect of different drugs, administered perioperatively, on tooth sensitivity and the degree of tooth whitening after in-office bleaching. A triple-blind,parallel design, randomized clinical trial were conducted. In the first experiment, the participants received a placebo or 400 mg ibuprofen every 8 h while in the second experiment, they were given a placebo or 60 mg etoricoxib every 24 h. In the third experiment, they received a placebo or 500 mg ascorbic acid every 8 h. In all experiments, the first dose of the drug was administered 1 h before in-office bleaching and taken for a period 48 h. The 35% hydrogen peroxide gel (Whiteness HP Maxx, FGM) was used in three 15-min applications for three groups following the manufacturer's directions. The in-office bleaching agent was refreshed every 15 min.Two bleaching sessions, with a one-week interval, were performed on each patient. The shade evaluation were performed before and 30-days, after bleaching with a Vita Easy Shade spectrophotometer and a Vita Classical’s visual shade guide. The tooth sensitivity was recorded on three scales: the five-point verbal rating scale, the 101 points scale, and the visual analogue scale, of and in different time points up to 1 h, 24 h, 48h after bleaching. The % of patients that reported TS at least once during treatment were evaluated by Fisher`s exact and the intensity of tooth sensitivity with Mann-Whitney test. Tooth color changes were evaluated by repeated ANOVA measures and t-test. There were no significant differences in the % of patients with tooth sensitivity and color change between the groups. The lowest tooth sensitivity was observed in the 400 mg ibuprofen group only up to 1 h post bleaching. The perioperative use of ibuprofen, etoricoxib and ascorbic acid per oral route was not able to avoid bleaching-induced tooth sensitivity according to the posology and dose used in this study. However, the 400 mg ibuprofen was able to reduce the intensity of tooth sensitivity up to 1 h post bleaching.

Neste estudo foram realizados três experimentos com o objetivo de determinar o efeito de dois anti-inflamatórios e um antioxidante utilizados previamente e durante o procedimento de clareamento dental em consultório na sensibilidade dental e no grau de clareamento alcançado. Estes experimentos foram do tipo estudo clínico, randomizado, paralelo, controlado por placebo e triplo cego. No experimento 1, os participantes receberam placebo ou ibuprofeno 400 mg a cada 8 h. No experimento 2, os participantes receberam placebo ou etoricoxibe 60 mg a cada 24 h. No experimento 3, os participantes receberam placebo ou ácido ascórbico 500 mg a cada 8 h. Em todos os experimentos os medicamentos foram administrados 1 hora antes do início do tratamento clareador por um período total de 48 h. Em cada sessão de clareamento, o gel de peróxido de hidrogênio 35% (Whiteness HP Maxx, FGM) foi aplicado 3 vezes durante 15 minutos cada, de acordo com as instruções do fabricante e repetido depois de uma semana, seguindo o mesmo protocolo. As alterações de cor foram avaliadas antes e 30 dias após o tratamento clareador utilizando o espectrofotômetro Vita Easy Shade e a escala de cor Vita Clássica organizada por ordem de valor. Três escalas destinadas a avaliar a intensidade da dor (escala verbal de 5 pontos, escala numérica de 101 pontos e escala visual analógica) foram utilizadas para determinar o nível de sensibilidade imediatamente até 1 h, 24 h e 48 h após o tratamento de clareamento em consultório. A porcentagem de pacientes que relataram sensibilidade, ao menos uma vez durante o tratamento, e a intensidade da sensibilidade dental foram avaliadas pelo teste exato de Fischer e teste de Mann-Whitney, respectivamente. As alterações de cor dental foram determinadas por medidas repetidas ANOVA 1 critério e teste t. Não foram detectadas diferenças significativas no percentual de pacientes que apresentaram sensibilidade e nem na alteração de cor. Somente o medicamento ibuprofeno 400 mg apresentou redução da intensidade da sensibilidade na primeira hora após o tratamento clareador. Os medicamentos não foram capazes de prevenir e nem mesmo minimizar a sensibilidade dental quando administrados de acordo com a posologia e dose empregadas neste estudo.Made available in DSpace on 2017-07-24T19:22:32Z (GMT).
