AVALIAÇÃO CLÍNICA DO CONDICIONAMENTO COM ÁCIDO ETILENODIAMINO TETRA-ACÉTICO NA ADESÃO COM UM ADESIVO AUTOCONDICIONANTE

Resumo: | Several adhesive strategies have emerged in order to improve performance of self-etch adhesives system, but to get an effective long-term adhesion in dentin substrate remains a challenge for these systems. It was recently suggested the use of EDTA as a dentin conditioning, which seems to have considerably improved the bond strength in laboratory tests. However, the results of laboratory studies cannot always predict the clinical behavior of materials for more complexes and elaborate that they can be. In this sense, it identifies the importance of a longitudinal clinical trial to confirm the effectiveness of this dentin conditioning associated to a self-etch adhesive. The aim of this study was to evaluate the clinical effectiveness of conditioning with EDTA, in adhesion with a self-etch adhesive system in non-carious cervical lesions [LCNC] for a period of up to 6 months. Ninety-six LCNC with features of scleroses were selected within forty-eight patients according to inclusion criteria. Each patient must have two non-carious cervical lesions with similar size, expelled and at least 50 of margins without enamel, with the neck located in dentin / cementum. Three calibrated operators unaware of the randomization procedures for the placement of restorations and conducting clinical evaluations in the time immediately and 6 months after placement of restorations. The criteria used for evaluation were the Federation Dental International (FDI): fracture / retention, marginal adaptation, postoperative sensitivity, and caries adjacent to restoration. Data were analyzed by distribution descriptive analyze, Fisher´s exact and Mc Nemar´s tests. Sensitivity was observed preoperatively in 63.5 of spontaneous LCNC, but in the immediate postoperative period no patient had a sensitivity, and evaluation of 6 months only 2.1. Just a restoration released, and another presented with a fracture of the margins to the time of evaluation, both in the experimental group without EDTA, achieving a retention rate of 100 in the experimental group and 95.7 in the control group, but no statistically significant difference was detected (p 0.05). Both groups had similar rates of marginal desadaptation criterion "B" FDI at 6 months (p 0.05), however, both showed significant difference compared to baseline (p 0.05). The present study demonstrates excellent clinical performance of the adhesive system Adper Easy One with and without use of EDTA after 6 months, and this adhesive system would have the provisional acceptance of the ADA. However assessments with longer longevity need to be performed. |

However assessments with longer longevity need to be performed.Inúmeras estratégias adesivas tem surgido com o intuito de melhorar o desempenho dos sistemas adesivos autocondicionantes, porém obter uma união efetiva no substrato dentinário que perdure ao longo do tempo continua sendo um desafio para estes materiais. Recentemente foi sugerida a utilização do EDTA como condicionante dentinário, devido ao fato desta substância melhor consideravelmente a resistência de união a dentina em testes laboratoriais. No entanto, por mais complexos e bem elaborados que os testes in vitro possam ser, os resultados deste tipo de estudo nem sempre consegue prever o comportamento clínico dos materiais. Nesse sentido, identifica-se a importância de um estudo clínico longitudinal que confirme a efetividade desta estratégia adesiva. Sendo assim, o objetivo desse trabalho foi avaliar a efetividade clínica do condicionamento com EDTA, na adesão com um adesivo autocondicionante em lesões cervicais não cariosas [LCNC] por um período de até 6 meses. Noventa e seis LCNC com características de esclerose foram selecionadas em quarenta e oito pacientes de acordo com os critérios de inclusão. Cada paciente devia ter duas lesões cervicais não cariosas com tamanhos semelhantes, expulsivas e com ao menos 50% das margens sem esmalte, com a região cervical localizada em dentina/cemento. Foram dispostos três operadores previamente calibrados que eram “cegos” em relação ao procedimento de aleatorização para a colocação das restaurações e realização das avaliações clínicas no tempo imediato e 6 meses após a colocação das restaurações. Os critérios utilizados para a avaliação foram os da Federação Dentária Internacional (FDI): fratura/retenção; adaptação marginal; sensibilidade pós- operatória; e cárie adjacente à restauração. Os dados foram submetidos a analise estatística descritiva para mostrar a distribuição de frequência. O teste exato de Fisher´s foi usado para mostrar a diferença entre os grupos e o desempenho de cada material ao longo do tempo foi avaliado pelo teste de Mc Nemar´s. Observou-se sensibilidade pré-operatória espontânea em 63,5% das LCNC, porém no pós-operatório imediato nenhum paciente apresentou sensibilidade, e na avaliação de 6 meses apenas 2,1% tiveram esta sintomatologia. Somente uma restauração soltou, e outra apresentou fratura das margens no momento da avaliação, ambas do grupo controle sem EDTA, obtendo uma taxa de retenção de 100% do grupo experimental com condicionamento com EDTA e 95,7% do grupo controle sem EDTA (p > 0,05). Ao teste exato de Fisher´s os dois grupos mostraram taxas similares de desadaptação marginal critério “B” FDI após 6 meses (p > 0,05), entretanto, ambos mostraram diferença significativa em relação ao baseline (p < 0,05). O presente estudo demonstra excelente desempenho clínico do sistema adesivo Adper Easy One com e sem uso do EDTA após 6 meses, sendo que este sistema adesivo teria a aceitação provisória da ADA.
