Gestão do cuidado de enfermagem à pessoa com câncer em hospital universitário: proposta de cuidado colaborativo

This is a qualitative research designed from the methodological framework of Convergent Care Research whose main objective was to investigate how the management of nursing care to people with hospitalized cancer is organized in a university hospital from the perspective of collaborative care. The first stage of the study was attended by 21 nurses from three hospitalization units. After approval by the Ethics Committee, conversation and interviews were conducted through a semi-structured script. Bardin's thematic content analysis supported the construction of relationships between themes that were grouped into 4 categories: "the perception of the profile of hospitalized patients and the problem associated with care practice"; "nursing care management for people with hospitalized cancer”; “the generalist nurse in the management of nursing care to the person with hospitalized cancer”; “analysis of the need for institutional support to the generalist nurse". Care management demonstrated weaknesses from the organizational and professional point of view due to the lack of planning and specific knowledge of the care team for specialized care, but opportunities for improvement were identified to overcome the limitations of generalist nurses through continuing education and the support of the oncology specialist. In the light of Patricia Benner's reference and the components of collaborative care, the collaborative management model of the care of the person with hospitalized cancer was elaborated, with emphasis on the role of the nurse. This model was applied to the situation of chemotherapy administration in the ward in order to enhance the collective reflection on its use. The complexity of oncological care in non-specialized spaces indicates the need for institutional support to non-specialist nurses, which permeates collaborative interprofessional actions and the commitment of all actors involved in the dimensions of care management.
